Function and Efficacy
Adrenal cortex hormone drugs. It has anti-inflammatory, anti-allergic, anti-rheumatic and immunosuppressive effects. Its mechanism of action is as follows: (1) Anti-inflammatory effect: This product reduces and prevents tissue response to inflammation, thereby reducing the manifestations of inflammation. It can inhibit the accumulation of inflammatory cells, including macrophages and leukocytes, at the site of inflammation, and inhibit phagocytosis, the release of lysosomal enzymes, and the synthesis and release of inflammatory chemical mediators. (2) Immunosuppressive effect: including preventing or inhibiting cell-mediated immune responses, delayed allergic reactions, reducing the number of T lymphocytes, monocytes, and eosinophils, reducing the ability of immunoglobulins to bind to cell surface receptors, and inhibiting the synthesis and release of interleukins, thereby reducing the transformation of T lymphocytes into lymphoblasts and reducing the expansion of primary immune responses. This product also reduces the passage of immune complexes through the basement membrane and can reduce the solubility of complement components and immunoglobulins.

Indication
Cortisone acetate tablets are mainly used to treat primary or secondary adrenocortical insufficiency, as well as various types of congenital adrenal hyperplasia caused by defects in the enzyme system required for the synthesis of glucocorticoids. If necessary, its pharmacological effects can also be used to treat a variety of diseases, including: 1. Autoimmune diseases, such as systemic lupus erythematosus, vasculitis, polymyositis, dermatomyositis, Still's disease, Graves' ophthalmopathy, autoimmune hemolysis, thrombocytopenic purpura, and myasthenia gravis. 2. Allergic diseases, such as severe bronchial asthma, anaphylactic shock, serum sickness, and atopic dermatitis. 3. Organ transplant rejection, such as kidney, liver, heart and other tissue transplants. 4. Inflammatory diseases, such as segmental ileitis, ulcerative colitis, and inflammatory eye diseases. 5. Blood diseases, such as acute leukemia and lymphoma
Usage and Dosage
Oral. For the treatment of adrenal cortex insufficiency, the daily dose for adults is generally 25-37.5 mg, 2/3 in the morning and 1/3 in the afternoon. When the patient is under stress (such as fever, infection), the dose should be increased appropriately. In case of severe stress, hydrocortisone should be changed to intravenous injection.
Adverse Reactions
1. Long-term use may cause Cushing syndrome. 2. Psychiatric symptoms such as excitement, restlessness, disorientation, depression, etc. 3. Concurrent infections: mainly fungi, tuberculosis, staphylococcus, Proteus, Pseudomonas aeruginosa, and various herpes viruses. 4. Post-discontinuation syndrome: hypothalamus; pituitary; adrenal insufficiency caused by fatigue, weakness, nausea, vomiting, low blood pressure, etc.
Precautions
This product is contraindicated for patients who are allergic to other steroid hormones. The following patients should generally avoid using it. In special circumstances, the pros and cons should be weighed before using it, and attention should be paid to the possibility of worsening of the condition: patients with gastrointestinal ulcers, glaucoma, electrolyte disorders, thrombosis, myocardial infarction, and visceral surgery.
Drug Interactions
1. Nonsteroidal anti-inflammatory analgesics can enhance its ulcerogenic effect. 2. It can enhance the hepatotoxicity of acetaminophen. 3. When used in combination with amphotericin B or carbonic anhydrase inhibitors, it can aggravate hypokalemia. Long-term use with carbonic anhydrase inhibitors can easily lead to hypocalcemia and osteoporosis. 4. When used in combination with protein anabolic hormones, it can increase the incidence of edema and aggravate acne. 5. When used in combination with anticholinergics (such as atropine) for a long time, it can cause increased intraocular pressure. 6. Tricyclic antidepressants can aggravate the mental symptoms caused by it. 7. When diabetic patients use this drug, their blood sugar may rise, and the dose of hypoglycemic drugs should be adjusted appropriately. 8. Thyroid hormones can increase its metabolic clearance rate, so when thyroid hormones or antithyroid drugs are used in combination with it, the dose of the latter should be adjusted appropriately. 9. When used in combination with contraceptives or estrogen preparations, its therapeutic effects and adverse reactions can be enhanced. 10. When used in combination with cardiac glycosides, it can increase the occurrence of digitalis toxicity and arrhythmia. 11 When used in combination with potassium-excreting diuretics, it can cause severe hypokalemia and weaken the sodium-excreting and diuretic effect of diuretics due to water and sodium retention. 12 When used in combination with ephedrine, its metabolic clearance can be enhanced. 13 When used in combination with immunosuppression, it can increase the risk of infection and may induce lymphoma or other lymphoproliferative diseases. 14 It can increase the metabolism and excretion of isoniazid in the liver, reducing the blood concentration and efficacy of isoniazid. 15 It can promote the metabolism of mexiletine in the body and reduce the blood concentration. 16 When used in combination with salicylates, it can reduce the concentration of plasma salicylate. 17 When used in combination with growth hormone, it can inhibit the latter's growth-promoting effect. The combination of glucocorticoids with ulcer-causing drugs (such as salicylates and NSAIDs) increases the risk of gastrointestinal complications. The combination of glucocorticoids with thiazide diuretics increases the risk of impaired glucose tolerance. Glucocorticoids increase the demand for insulin and oral hypoglycemic drugs in diabetic patients. Patients taking corticosteroids should not receive vaccinia vaccination or other immunizations, especially those taking high doses, because of the risk of neurological complications and/or lack of antibody response. Corticosteroids should be used with caution in patients with hypoprothrombin when combined with acetylsalicylic acid. Convulsions have been reported with the concomitant use of methylprednisolone and cyclosporine. Because the two drugs inhibit each other's metabolism, seizures and other side effects that occur with either drug are more likely to occur with the concomitant use of both drugs.
