Function and Efficacy

The main pharmacological action is to relax vascular smooth muscle. Nitroglycerin releases nitric oxide (NO), which is the same as endothelial relaxing factor, activates guanylate cyclase, increases cyclic guanosine monophosphate (cGMP) in smooth muscle and other tissues, leads to dephosphorylation of myosin light chain, regulates the contractile state of smooth muscle, and causes vasodilation. Nitroglycerin dilates the arteriovenous vascular bed, mainly dilating the veins, and its effect intensity is dose-related. Peripheral vein dilation causes blood to remain in the periphery, reducing the amount of blood returning to the heart and reducing left ventricular end-diastolic pressure (preload). Dilation of arteries reduces peripheral resistance (afterload). Arteriovenous dilation reduces myocardial oxygen consumption and relieves angina pectoris. It also has a dilating effect on epicardial coronary artery branches. Therapeutic doses can reduce systolic blood pressure, diastolic blood pressure, and mean arterial pressure, and effectively relax coronary arteries.

Appearance

This product is white tablets.

Indication

It is used for the treatment and prevention of angina pectoris caused by coronary heart disease, and can also be used to lower blood pressure or treat congestive heart failure.

Usage and Dosage

Tablets: Adults take 0.25-0.5 mg (1 tablet) sublingually at a time. Repeat 1 tablet every 5 minutes until the pain is relieved. If the pain persists after taking 3 tablets in 15 minutes, seek medical attention immediately. Use it prophylactically 5-10 minutes before activities or bowel movements to avoid inducing angina pectoris.

Adverse Reactions

(1) Headache: may occur immediately after medication, may be severe and persistent. (2) Dizziness, weakness, palpitations and other manifestations of postural hypotension may occasionally occur, especially in patients who are standing or immobilized. (3) Significant hypotension reactions may occur at therapeutic doses, manifested as nausea, vomiting, weakness, sweating, pallor and collapse. (4) Syncope, flushing, drug rash and exfoliative dermatitis have all been reported.

Precautions

It is contraindicated in patients with early myocardial infarction (with severe hypotension and tachycardia), severe anemia, glaucoma, increased intracranial pressure, and known allergy to nitroglycerin. It is also contraindicated in patients using sildenafil citrate (Viagra), which enhances the antihypertensive effect of nitroglycerin.

Special Population Medication

Precautions for children: Not yet clear. Precautions for pregnancy and lactation: It is not known whether it will cause fetal damage or affect fertility, so it should only be used in pregnant women when it is necessary. It is also not known whether it is excreted from human breast milk, so lactating women should be cautious. Precautions for the elderly: Not yet clear.

Drug Interactions

1. When drinking moderately or excessively, the use of this drug may cause hypotension. 2. Combination with antihypertensive drugs or vasodilators may enhance the postural hypotensive effect of nitrates. 3. Aspirin can reduce the clearance of sublingual nitric acid and glycerol and enhance its hemodynamic effects. 4. The use of long-acting nitrates may reduce the therapeutic effect of sublingual medications. 5. Sildenafil citrate (Viagra) enhances the antihypertensive effect of organic nitrates. 6. When used in combination with acetylcholine, histamine and sympathomimetic amines, the efficacy may be weakened.
