Urapidil Injection

Urapidil is a highly selective α1 receptor blocker with both peripheral and central effects. The peripheral vasodilator effect is mainly to block postsynaptic α1 receptors, which significantly reduces peripheral resistance. At the same time, it also has a moderate vasoconstrictor effect of blocking catecholamines. The central effect is to reduce blood pressure by stimulating serotonin-1A receptors and reducing the sympathetic feedback regulation of the medullary cardiovascular center. At the same time, it can also improve various hemodynamic indicators by reducing the side effect of reflex heart rate acceleration.

This product is a colorless clear liquid.

It is used to treat hypertensive crisis (such as a sharp rise in blood pressure), severe and extremely severe hypertension, and refractory hypertension. It is used to control perioperative hypertension.

Intravenous injection: Slowly inject 10-50 mg intravenously and monitor the changes in blood drugs. The antihypertensive effect should be shown within 5 minutes. If the effect is not satisfactory, the medication can be repeated. Continuous intravenous drip or infusion pump: After intravenous injection, in order to maintain its antihypertensive effect, this product can be continuously intravenously dripped. The liquid is prepared as follows: Usually 250 mg of urapidil is added to a suitable liquid, such as normal saline, 5% or 10% glucose, 5% fructose or dextran 40 plus 0.9% sodium chloride solution. If an infusion pump is used to maintain the dose, 100 mg of urapidil can be added and the above liquid can be diluted to 50 ml. The maximum drug concentration of intravenous infusion is 4 mg of urapidil per milliliter. The infusion rate is determined according to the patient's blood pressure.

After using this product, patients may experience headache, dizziness, nausea, vomiting, sweating, irritability, fatigue, palpitations, arrhythmia, upper chest oppression or dyspnea, etc., which are mostly caused by too rapid a drop in blood pressure. Usually, they disappear within a few minutes, and patients do not need to stop taking the drug. Allergic reactions are rare (such as itching, redness of the skin, rash, etc.). In very rare cases, platelet counts decrease when taking this drug orally, but serum immunology studies have not yet confirmed the causal relationship.

It is contraindicated in patients who are allergic to the ingredients of this product. It is contraindicated in patients with aortic isthmus stenosis or arteriovenous shunt (except for shunt during renal dialysis). It is contraindicated in breastfeeding women.

Precautions for children: Children rarely use this drug, and there is currently a lack of data in this regard. Precautions for pregnancy and lactation: It is contraindicated for lactating women. Precautions for the elderly: Elderly patients must use antihypertensive drugs with caution, and the initial dose should be small, because their sensitivity to drugs is sometimes difficult to estimate.

If the patient is using other antihypertensive drugs, drinking alcohol, or has hypovolemia (such as diarrhea, vomiting), the antihypertensive effect of this product may be enhanced. The simultaneous use of cimetidine may increase the blood concentration of this product by up to 15%.

Keep in a sealed container. The validity period is tentatively set at two years.
