Timolol Maleate Eye Drops

Timolol maleate is a non-selective beta-adrenergic receptor blocker with no obvious endogenous sympathomimetic activity and local anesthetic effect, and no direct inhibitory effect on the myocardium. This product is timolol maleate eye drops, which can reduce intraocular pressure in patients with high intraocular pressure and normal people. The exact mechanism of its lowering intraocular pressure is still unclear. Intraocular pressure tracing and aqueous humor fluorescence photometry studies suggest that the intraocular pressure-lowering effect of this product is related to reducing aqueous humor production.

The main ingredient is timolol, and its chemical name is (I)-1-(tert-butylamino)-3-[(4-morpholino-1,2,5-thiadiazol-3-yl)oxy]-2-propanol maleate.

This product is a colorless clear liquid.

It has a good therapeutic effect of reducing intraocular pressure for primary open-angle glaucoma. For aphakic glaucoma, some secondary glaucoma, ocular hypertension, and glaucoma that is ineffective with other drugs and surgery, the addition of this product to eye drops can further enhance the antihypertensive effect. It is also used clinically for hypertension.

Eye drops, 1 drop at a time, 1 to 2 times a day. If the intraocular pressure is under control, it can be changed to once a day. If other drugs are used, when switching to this product for treatment, the original drug should not be stopped suddenly. It should be gradually stopped from the second day of using this product.

1 The most common adverse reactions are burning and stinging of the eyes. 2 Cardiovascular system: bradycardia, arrhythmia. 3 Nervous system: dizziness, aggravation of myasthenia gravis symptoms, paresthesia, drowsiness, insomnia, nightmares, depression, mental confusion, hallucinations. 4 Respiratory system: bronchospasm, respiratory failure, dyspnea, nasal congestion, cough, upper respiratory tract infection. 5 Endocrine system: masking the symptoms of hypoglycemia in diabetic patients after taking insulin or hypoglycemic drugs.

1. Patients with bronchial asthma or a history of bronchial asthma, severe chronic obstructive pulmonary disease. 2. Sinus bradycardia, Ⅱ or Ⅲ degree atrioventricular block, obvious heart failure, cardiogenic shock. 3. Patients who are allergic to this product.

1. Combined use with adrenaline can cause pupil dilation. 2. It is not recommended to use two local beta-receptor blockers at the same time. 3. This product should be used with caution in combination with calcium channel antagonists, as it can cause atrioventricular conduction block, left ventricular failure and hypotension. For patients with impaired cardiac function, the two drugs should be avoided in combination. 4. Patients who are taking catecholamine-depleting drugs (such as reserpine) should be closely observed when using this product, as it can cause hypotension and significant bradycardia. 5. This product can further prolong atrioventricular conduction time when used in combination with digitalis and calcium channel antagonists. 6. This product can cause side effects of systemic beta-receptor blockade such as slowing of heart rate when used in combination with quinidine. The possible reason is that quinidine can inhibit the metabolism of timolol by P450 enzymes and CYPZD6.
