Amrinone injection

Function and Efficacy

This product is a phosphodiesterase inhibitor, and its mechanism of action has not been fully elucidated. Experiments have shown that this product has positive inotropic and vasodilatory effects. The positive inotropic effect is mainly through the inhibition of phosphodiesterase, which increases the concentration of cyclic adenosine monophosphate (CAMP) in myocardial cells, increases intracellular calcium, strengthens myocardial contractility, and increases cardiac output. It has nothing to do with adrenaline-1 receptors or myocardial cell Na, K-ATPase. Its vasodilatory effect may be directly acting on small arteries, or after the heart function is improved, the excitement of the sympathetic nerves is reduced, thereby reducing the heart's preload and afterload, reducing the left ventricular filling pressure, improving the left ventricular function, and increasing the cardiac index, but it has no significant effect on the mean arterial pressure and heart rate.

Ingredients

The chemical name of this product is: 5-amino-[3,4'bipyridine]-6(1H)-one. Molecular formula: C10H9N3O Molecular weight: 187.2

Appearance

This product is a light yellow or yellow clear liquid.

Indication

It is suitable for acute and chronic refractory congestive heart failure caused by various reasons that are ineffective or poorly treated with digitalis, diuretics and vasodilators.

Usage and Dosage

50mg of this product is diluted with an appropriate amount of normal saline before use. Loading dose: 0.5-1.0mg/kg, slowly injected intravenously over 5-10 minutes, and then continued with 5-10?g/(kg·min) intravenous drip, with a single dose of no more than 2.5mg/kg. The maximum daily dose is less than 10mg/kg. The course of treatment should not exceed 2 weeks. During the application period, the toxicity of digitalis is not increased, the myocardial oxygen consumption is not increased, and there is no sign of increased myocardial ischemia in ischemic heart disease, so there is no need to stop using digitalis, diuretics and vasodilators.

Adverse Reactions

There may be gastrointestinal reactions, thrombocytopenia (2 to 4 weeks after medication), ventricular arrhythmias, hypotension, and liver and kidney damage. Occasionally, it may cause allergic reactions, fever, rash, chest pain, hematemesis, myalgia, mental symptoms, phlebitis, and local irritation after injection. Long-term oral administration has great side effects and may even increase mortality. Oral preparations are no longer used and are limited to short-term intravenous use for refractory heart failure.

Precautions

Severe hypotension.

Drug Interactions

1. When used together with disopyramide, it may lead to hypotension. 2. When used together with commonly used cardiotonic, diuretic and vasodilator drugs, no adverse interactions have been observed. 3. When used together with nitrates, it has an additive effect. 4. This product enhances the positive inotropic effect of digitalis, so it is not necessary to stop using digitalis during its use.

Storage

Keep away from light and store in a sealed container.
