Function and Efficacy

1. Increase the reabsorption of calcium by the small intestine and renal tubules, inhibit parathyroid hyperplasia, reduce the synthesis and release of parathyroid hormone, and inhibit bone resorption; 2. Increase the synthesis of transforming growth factor-β (TGF-β) and insulin-like growth factor-I (IGF-I), promote the synthesis of collagen and bone matrix protein; 3. Regulate muscle calcium metabolism, promote muscle cell differentiation, enhance muscle strength, increase neuromuscular coordination, and reduce the tendency to fall.

Usage and Dosage

Oral administration: Adults, 0.25-1 μg per day.

Adverse Reactions

Small doses alone (<1.0 μg/d) generally have no adverse reactions, but long-term high-dose use or combined use with calcium supplements may cause hypercalcemia and hypercalciuria.

Precautions

It is contraindicated for patients who are allergic to vitamin D and its analogs, have hypercalcemia, or have signs of vitamin D poisoning.

Special Population Medication

Precautions for children: See dosage and administration, or follow the doctor's advice. Precautions for pregnancy and lactation: There is not enough evidence for the safety of taking alfacalcidol during pregnancy. Although animal tests have shown that it is harmless, like other drugs, alfacalcidol can be used only when it is needed during pregnancy and there are no other alternatives. The safety of medication during lactation has not yet been finalized, but when taking alfacalcidol, the content of 1,25-dihydroxyvitamin D3 in breast milk may increase. Since this will affect the calcium metabolism of infants, the drug should be discontinued during lactation. Precautions for the elderly: See dosage and administration, precautions, etc., or follow the doctor's advice.

Drug Interactions

1 Calcium supplements: Combination with calcium supplements may cause elevated blood calcium, so blood calcium should be monitored. 2 Thiazide diuretics: This type of diuretic can promote the kidney's absorption of calcium. There is a risk of hypercalcemia when used together. 3 Digitalis glycosides: Hypercalcemia in patients taking digitalis drugs can easily induce arrhythmias. If this drug is used in combination, blood calcium should be closely monitored. 4 Barbiturates, anticonvulsants: These drugs can accelerate the metabolism of active vitamin D metabolites in the liver and reduce the efficacy of the drug, so the dose of this drug should be appropriately increased. 5 Gastrointestinal absorption inhibitors: Cholestyramine or aluminum-containing antacids can reduce the absorption of this drug. The two should not be taken at the same time. The drugs should be taken one after the other with an interval of 2 hours. <b
