Compound Metformin Glipizide Capsules
Function and Efficacy
1. Pharmacology Metformin hydrochloride does not promote insulin secretion. Its mechanism of hypoglycemic action is to promote tissue anaerobic glycolysis, so that muscle and other tissues can use glucose more effectively, while inhibiting hepatic glycogen synthesis, reducing the production of glycogen, and lowering blood sugar. Glipizide is mainly used as a second-generation sulfonylurea antidiabetic drug. It is effective for most patients with type 2 diabetes, and can reduce fasting and postprandial blood sugar, and reduce glycosylated hemoglobin (HbA2C) by 1%-2%. The main function of this type of drug is to stimulate pancreatic B cells to secrete insulin, but the prerequisite is that pancreatic B cells still have a certain function of synthesizing and secreting insulin. Its mechanism is to specifically bind to the xanthourea receptor on the B cell membrane, thereby closing the K channel, causing changes in membrane potential, opening the Ca2 channel, and increasing intracytoplasmic Ca2, which promotes insulin secretion. In addition, there are extra-pancreatic effects, including improving the insulin resistance of peripheral tissues (such as liver, muscle, and fat). 2. Toxicity: The oral LD50 of mice is 6.98 mg/g, and the intraperitoneal injection LD50 of mice is 6.01 mg/g. This product has no in vitro hemolytic reaction and has no irritation to the gastrointestinal tract of rabbits and mice.

Appearance
The content of this product is off-white or light yellow powder.
Indication
It is used for patients with mild, moderate and elderly non-insulin-dependent diabetes who have not achieved satisfactory results after 2-3 months of diet control and physical exercise.
Usage and Dosage
Oral administration: 1-2 capsules at a time, 2-3 times a day, or as directed by your doctor.
Adverse Reactions
1. The most common symptoms are gastrointestinal symptoms (such as nausea, upper abdominal distension) and headache, which can be relieved by reducing the dosage. 2. Some patients may experience skin allergies. 3. Hypoglycemia is occasionally seen, especially in the elderly and infirm, those who are overactive, those who eat irregularly, drink alcohol, or have liver damage. 4. There are also occasional reports of reversible changes in the hematopoietic system. 5. Although the incidence of lactic acidosis is very low, it should be paid attention to. Clinical manifestations include vomiting, abdominal pain, hyperventilation, mental disorders, and increased lactic acid concentration in the blood that cannot be explained by uremia, painful acidosis or salicylic acid poisoning. 6. It can reduce the intestinal absorption of vitamin B12, reduce hemoglobin, produce megaloblastic anemia, and may also cause malabsorption.
Precautions
1. Non-islet-dependent diabetes mellitus with ketosis, acidosis, hepatic and renal insufficiency, heart failure, acute myocardial infarction, severe infection, major surgery, sepsis, and clinical hypotension and hypoxia are contraindicated. 2. Excessive drinkers, dehydration, dysentery, and malnutrition are contraindicated. 3. Those who are allergic to this product, biguanides, and sulfonamides are contraindicated. 4. Patients with a clear diagnosis of type 1 diabetes are contraindicated. 5. Patients with hepatic and renal insufficiency are contraindicated. 6. Patients with leukopenia are contraindicated. 7. Pregnant and lactating women are contraindicated.
Special Population Medication
Precautions for children: Not clear yet. Precautions for pregnancy and lactation: 1. Animal experiments and clinical observations have shown that sulfonylurea hypoglycemic drugs can cause stillbirth and fetal malformation, so pregnant women are prohibited from using them. 2. This type of drug can be excreted through breast milk, so nursing mothers should not use it to avoid hypoglycemia in infants. Precautions for the elderly: 1. Start with a small dose and gradually adjust the dose. 2. Patients over 70 years old may experience lactic acidosis, so use with caution.
Drug Interactions
1. The combination of this drug and insulin will enhance the hypoglycemic effect, so it is advisable to reduce the insulin dose. 2. It can enhance the anticoagulant effect of anticoagulants (such as warfarin), leading to a bleeding tendency. 3. If this product is taken with alcoholic beverages, abdominal pain, acidosis and hypothermia may occur. 4. The combination of this drug with dicoumarols, monoamine oxidase inhibitors, phenylbutazone, sulfonamides, chloramphenicol, cyclophosphamide, probenecid, and salicylic acid drugs can increase its hypoglycemic effect. 5. When used in combination with epinephrine, adrenocortical hormones, oral contraceptives, and thiazide diuretics, its hypoglycemic effect can be reduced. 6. Be cautious when used in combination with beta-receptor blockers. 7. Gastrointestinal diseases that shorten the retention time of this product in the gastrointestinal tract can affect the pharmacokinetics and efficacy of this product.
