Yiduozhi capsules
Function and Efficacy
This product is a blood lipid regulating drug of clofibric acid derivatives. Its mechanism of lowering blood lipids is not yet fully understood. It may reduce the cAMP content in liver microsomes, increase the activity of lipoprotein esterase, and decompose lipids in lipoproteins. Experimental and clinical studies have shown that this product reduces blood cholesterol and triglycerides and increases blood high-density lipoprotein. In addition, this product also has anti-platelet aggregation and anti-thrombotic effects and reduces blood uric acid.
Ingredients
The main ingredients and chemical names of this product are: 2-methyl-2-(4-chlorophenoxy)-propionic acid, 2-(1,2,3,6-tetrahydro-1,3-dimethyl-2,6-dioxo-7H-purinyl)ethyl ester. Molecular formula: C19H21ClN4O5 Molecular weight: 420.85
Appearance
This product is the contents of capsules.
Indication
It is used to treat hyperlipidemia in adults who do not respond well to dietary control therapy. Its triglyceride-lowering effect is stronger than its cholesterol-lowering effect.
Usage and Dosage
This product should be taken orally after or during meals. If it is ineffective after taking this product for 3 months, it should be discontinued. The usual dosage for adults is 0.25g orally once, twice a day, which can be increased to 0.25g once, 3 times a day with the treatment response. If combined with hyperuricemia, 0.25g once, 3 times a day can be taken at the beginning. After treatment, if blood lipids are normal, 0.25g once, 2 times a day can be used to maintain.
Adverse Reactions
1The most common adverse reactions are gastrointestinal discomfort, such as indigestion, nausea, fullness, stomach discomfort, etc. Other less common adverse reactions include headache, dizziness, fatigue, rash, itching, impotence, anemia and leukopenia. Some cases have elevated blood aminotransferase. 2This product is a derivative of clofibric acid, which may cause myositis, myopathy and rhabdomyolysis syndrome, leading to elevated blood creatine phosphokinase; rhabdomyolysis occurs, mainly manifested by myalgia combined with elevated blood creatine phosphokinase, myoglobinuria, and can lead to renal failure, but it is rare; in patients with nephrotic syndrome and other renal damage that lead to decreased blood albumin or patients with hyperthyroidism, the risk of myopathy increases. 3. This product increases the risk of cholelithiasis. Clofibric acid derivatives can affect the metabolism of steroids in three ways. They can inhibit the synthesis of cholesterol, inhibit the synthesis of bile acids, and enhance the excretion of cholesterol from bile. The latter two factors can increase the cholesterol saturation in bile, thus may cause gallstones to form in some patients, but there may also be other influencing factors.
Precautions
1. Patients who are allergic to edoxalate are contraindicated. 2. Patients with hepatic insufficiency or primary biliary cirrhosis are contraindicated because this product can promote increased cholesterol excretion and increase the already high cholesterol level. 3. Patients with severe renal insufficiency are contraindicated because taking this product may cause rhabdomyolysis and severe hyperkalemia in patients with renal insufficiency; patients with nephrotic syndrome causing hypoalbuminemia are contraindicated because they have an increased risk of myopathy. 4. Patients with gallbladder disease and cholelithiasis are contraindicated because this product can aggravate the symptoms of gallbladder disease and require surgery.
Special Population Medication
Precautions for children: There is insufficient research on the use of this product in children, and the pros and cons must be weighed when using it. Precautions for pregnancy and lactation: Pregnant and lactating women should not take this product. Precautions for the elderly: The elderly should adjust the dosage according to the liver and kidney function status.
Drug Interactions
1. The combined use of clofibric acid derivatives and HMG-CoA reductase inhibitors, such as pravastatin and simvastatin, to treat hyperlipidemia will increase the risk of severe muscle toxicity of both drugs, and their combined use should be avoided as much as possible. 2. When this product is used in combination with other drugs with high protein binding rates, they can also be replaced from the protein binding site, resulting in enhanced effects, such as tolbutamide and other sulfonylurea hypoglycemic drugs, phenytoin, furosemide, etc. If the above drugs are taken during lipid-lowering treatment, the doses of hypoglycemic drugs and other drugs should be adjusted. 3. This product can enhance the effect of oral anticoagulants. When used with it, attention should be paid to reducing the dose of oral anticoagulants. Its mechanism of action is still uncertain, but it may be because this product can replace warfarin and other drugs from their protein binding sites, thereby enhancing their effects. 4. When this product is used in combination with immunosuppressants, such as cyclosporine, it can increase the latter's blood drug concentration and nephrotoxicity. This product should also be used with other nephrotoxic drugs. 5. This product contains theophylline, and attention should be paid to reactions when used with theophylline.
