Function and Efficacy

Tizanidine is a central alpha 2 adrenergic receptor agonist, which may reduce tonic spasm by enhancing the presynaptic inhibition of motor neurons. Animal experiments have shown that tizanidine has no direct effect on skeletal muscle fibers and neuromuscular junctions, and has a weak effect on monosynaptic spinal reflexes. Tizanidine has the strongest effect on multisynaptic pathways, and these effects are believed to be related to the reduced facilitation of spinal motor neurons.

Indication

Tizanidine hydrochloride tablets are centrally acting skeletal muscle relaxants used to reduce increased skeletal muscle tension, muscle spasms and muscle rigidity caused by brain and spinal cord trauma, cerebral hemorrhage, encephalitis and multiple sclerosis.

Usage and Dosage

Patients should have a 2-4 week dose adjustment period for the first use. The initial dose is 2-4 mg each time, once every 6-8 hours. The single dose should not exceed 8 mg, and the daily dose should not exceed 24 mg. The maximum dose is 36 mg per day. Because this product has a strong first-pass effect when taken orally, attention should be paid to individualized dosage when using it.

Adverse Reactions

At low doses for painful muscle spasms, only mild transient drowsiness, fatigue, dizziness, dry mouth, nausea and a slight drop in blood pressure are observed. When used for spastic paralysis, the above adverse reactions are more common and more obvious due to the large dose, but there is no need to stop the drug. The specific adverse reactions are as follows. 1. Serious adverse reactions (the probability of occurrence is unknown) 1. Shock (hypotension, bradycardia, pale complexion, cold sweat, dyspnea, etc.) (In this case, the drug should be stopped immediately and appropriate treatment methods should be taken) 2. Precautions

Special Population Medication

Precautions for children: The safety of the drug for children has not been fully evaluated. Precautions for pregnancy and lactation: The use of this product in pregnant and lactating women has not been fully evaluated. It is not known whether the drug is secreted through breast milk, but due to the fat solubility of the drug, it may enter breast milk, so pregnant and lactating women should consider using it after fully weighing the pros and cons. Precautions for the elderly: This product is mainly excreted in the kidneys, so in many cases of low kidney function, for elderly patients, this drug can cause the blood drug concentration to continue to increase, so pay attention to reducing the dosage; this product has the effect of lowering blood pressure, and elderly patients should use it with caution.

Drug Interactions

In vitro experimental studies using human liver microsomal cytochrome P450 isozymes show that tizanidine hydrochloride and its metabolites do not affect the metabolism of other drugs by this enzyme. Tizanidine hydrochloride postpones the peak time of paracetamol by 16 minutes, and paracetamol has no effect on the pharmacokinetic parameters of tizanidine hydrochloride. Ethanol increases the area under the curve of tizanidine hydrochloride by about 20%, increases the maximum peak concentration by 15%, and increases the adverse reactions of tizanidine hydrochloride at the same time, and the central nervous system depressant effect of ethanol and tizanidine hydrochloride has an additive effect. A retrospective study of 4mg tizanidine hydrochloride given once or multiple times shows that, compared with patients who do not take oral contraceptives at the same time, taking oral contraceptives at the same time reduces the clearance rate of tizanidine hydrochloride by 50%.
