Function and Efficacy

This product can simultaneously relax the coronary arteries in the normal blood supply area and the ischemic area, antagonize spontaneous or ergonovine-induced coronary artery spasm, increase the delivery of myocardial oxygen in patients with coronary artery spasm, and relieve and prevent coronary artery spasm. It can also inhibit myocardial contraction, reduce myocardial metabolism, and reduce myocardial oxygen consumption. On the other hand, it can relax peripheral resistance vessels, reduce peripheral resistance, reduce systolic and diastolic blood pressure, and reduce cardiac afterload. This product can delay the sinus node function and atrioventricular conduction of isolated hearts; electrophysiological studies of whole animals and humans have not found that this product has the effect of delaying atrioventricular conduction, prolonging the recovery time of the sinus node, and slowing down the sinus node rate.

Indication

It is clinically applicable to the prevention and treatment of angina pectoris caused by coronary heart disease, especially variant angina pectoris and angina pectoris caused by coronary artery spasm. It is applicable to various types of hypertension and has a good effect on refractory and severe hypertension.

Usage and Dosage

The initial dose is 5 mg bid, the maximum dose is 10 mg tid. In case of angina attack or hypertension emergency, 5 mg can be taken sublingually or chewed. Oral: 5-10 mg once, 3 times a day. It can be taken sublingually in case of emergency. For chronic heart failure, 20 mg every 6 hours. Spray in the pharynx: 1.5-2 mg each time (about 3-4 sprays).

Adverse Reactions

Headache, flushing of the face and skin, hot flashes, tachycardia, palpitations, dizziness, fatigue. Hypotension. Swelling of the calves and ankles. Gastrointestinal disturbances such as nausea and diarrhea in extreme cases. Skin reactions such as itching, urticaria, and rashes. Long-term use in elderly male patients may cause gynecomastia. Long-term use may also cause gingival hyperplasia. May impair the ability to drive and operate machinery. Overdose may cause bradycardia and hypotension. Some cases may cause numbness at the root of the tongue, dry mouth, sweating, headache, nausea, and loss of appetite.

Precautions

Patients with hypotension, allergy to dihydropyridine drugs, cardiovascular shock, acute stage of myocardial infarction (within 8 days).

Special Population Medication

Precautions for children: Not clear yet. Precautions for pregnancy and lactation: 1. No detailed clinical research data. Nifedipine is clinically used for pregnant women with hypertension. 2. Nifedipine can be secreted into breast milk, so lactating women should stop taking the drug or stop breastfeeding. Precautions for the elderly: The half-life of nifedipine is prolonged in the elderly, so pay attention to adjusting the dose when using it.

Drug Interactions

Combination with other antihypertensive drugs can enhance the antihypertensive effect. Severe hypotension may occur when used simultaneously with B-receptor blockers. When used simultaneously with digoxin, digoxin clearance may be reduced. When taken simultaneously with quinidine, quinidine concentration decreases. Cimetidine or ranitidine may slightly increase the concentration of this drug.
