Function and Efficacy
Nifedipine is a dihydropyridine calcium antagonist that selectively inhibits the transmembrane transport of calcium ions into myocardial cells and smooth muscle cells, and inhibits the release of calcium ions from intracellular pools without changing the plasma calcium ion concentration. Pharmacological action: 1 This product can simultaneously relax the coronary arteries in the normal blood supply area and the ischemic area, antagonize spontaneous or ergonovine-induced coronary artery spasm, increase the delivery of myocardial oxygen in patients with coronary artery spasm, and relieve and prevent coronary artery spasm. 2 This product can inhibit myocardial contraction, reduce myocardial metabolism, and reduce myocardial oxygen consumption. 3 This product can relax peripheral resistance vessels, reduce peripheral resistance, and can reduce systolic and diastolic blood pressure, reducing cardiac afterload. 4 This product can delay the sinus node function and atrioventricular conduction of isolated hearts; electrophysiological studies on whole animals and humans have not found that this product has the effect of delaying atrioventricular conduction, prolonging the recovery time of the sinus node, and slowing down the sinus node rate. Carcinogenicity, mutagenicity and reproductive toxicity No carcinogenicity. No mutagenicity. Large doses can reduce the fertility of female mice; can cause teratogenesis; can cause miscarriage (increased drug absorption rate in fetal mice, increased fetal mortality, and decreased survival rate of newborn mice). Pregnant monkeys taking 2/3-2 times the maximum human dose can cause small placenta and chorionic dysplasia; giving rats 3 times the maximum human dose can cause prolonged pregnancy. The impact on human fertility is still unclear.

Indication
Severe angina pectoris, especially variant angina pectoris, and hypertension.
Usage and Dosage
1 The dose of nifedipine should be gradually adjusted according to the patient's tolerance and control of angina pectoris. Overdose of nifedipine can cause hypotension. 2 Start with a small dose, usually 10 mg/time, 3 times a day; the commonly used maintenance dose is 10-20 mg/time, 3 times a day. Some patients with obvious coronary spasm can use 20-30 mg/time, 3-4 times a day. The maximum dose should not exceed 120 mg/day. If the condition is urgent, 10 mg/time can be chewed or taken sublingually, and the decision to re-administer the drug depends on the patient's response to the drug. 3 It usually takes 7-14 days to adjust the dose. If the patient's symptoms are obvious and the condition is urgent, the dose adjustment period can be shortened. Depending on the patient's response to the drug, the frequency of attacks and the dose of sublingual nitroglycerin, the dose of nifedipine can be adjusted from 10-20 mg to 30 mg/time, 3 times a day within 3 days. 4 For hospitalized patients under strict monitoring, the dose can be increased by 10 mg every 4 to 6 hours, depending on the control of angina pectoris or ischemic arrhythmias.
Adverse Reactions
1. Common peripheral edema after taking the drug (peripheral edema is related to the dose, the incidence is 4% when taking 60mg/day, and 12.5% when taking 120mg/day); dizziness; headache; nausea; fatigue and facial flushing (10%). Transient hypotension (5%), most of which do not require discontinuation of the drug (transient hypotension is related to the dose, the incidence is 2% when taking 60mg/day, and 5% when taking 120mg/day). Some patients may experience angina pectoris, which may be related to the hypotensive reaction. Palpitations; nasal congestion; chest tightness; shortness of breath; constipation; diarrhea; gastrointestinal spasm; abdominal distension; skeletal muscle inflammation; joint stiffness; muscle spasm; mental tension; tremor; nervousness; sleep disorders; blurred vision; imbalance, etc. (2%). Syncope (0.5%), which will no longer occur if the dose is reduced or used in combination with other anti-anginal drugs. 2. Rare anemia; leukopenia; thrombocytopenia; purpura; allergic hepatitis; gingival hyperplasia; depression; paranoia; instant blindness at peak blood concentration; erythromelalgia; antinuclear antibody positive arthritis, etc. (0.5%). 3. Possible serious adverse reactions: myocardial infarction and congestive heart failure incidence rate 4%; pulmonary edema incidence rate 2%; arrhythmia and conduction block incidence rate less than 0.5% each. 4. Allergic to this product may cause allergic hepatitis, rash, and even exfoliative dermatitis, etc.
Precautions
It is contraindicated in patients allergic to nifedipine.
Drug Interactions
1 Nitrates are used in combination with this product to control angina attacks and have good tolerance. 2 Most patients with beta-receptor blockers have good tolerance and efficacy when using this product in combination, but some patients may induce and aggravate hypotension, heart failure and angina. 3 Digitalis This product may increase the blood digoxin concentration, suggesting that the blood concentration of digoxin should be monitored when using it for the first time, adjusting the dose or stopping this product. 4 When drugs with high protein binding rates such as dicoumarols, phenytoin sodium, quinidine, quinine, warfarin, etc. are used with this product, the free concentration of these drugs often changes. 5 When cimetidine is used with this product, the peak plasma concentration of this product increases, so pay attention to adjusting the dose. 6 Patients with fentanyl anesthesia undergoing coronary artery bypass grafting (or other surgeries), taking nifedipine alone or in combination with beta-receptor blockers can cause severe hypotension. If conditions permit, the drug should be discontinued for at least 36 hours.
