Function and Efficacy

Pharmacology: The main pharmacological effect of isosorbide dinitrate (ISDN) is to relax vascular smooth muscle. ISDN is metabolized in the body to form isosorbide mononitrate (ISMN), which releases nitric oxide (NO). NO is the same as endothelial relaxing factor, activating guanylate cyclase, increasing cyclic guanosine monophosphate (cGMP) in smooth muscle cells, thereby relaxing vascular smooth muscle, dilating peripheral arteries and veins, and having a stronger dilating effect on veins. Venous dilation causes blood to remain in the periphery, reducing the amount of blood returning to the heart, and reducing left ventricular end-diastolic pressure and pulmonary capillary wedge pressure (preload). Arterial dilation reduces peripheral vascular resistance, systolic arterial pressure, and mean arterial pressure (afterload). Coronary artery dilation increases coronary perfusion. The overall effect is to reduce myocardial oxygen consumption, increase oxygen supply, and relieve angina pectoris. Toxicology: No carcinogenicity or mutagenicity was observed in animal experiments. Appearance

This product is off-white or light yellow tablets.

Indication

Used to prevent, relieve or treat various types of angina pectoris; can also be used in combination with digitalis drugs and (or) diuretics to treat chronic congestive heart failure.

Usage and Dosage

Oral administration: 20 mg once, every 8 to 12 hours. Dosage increase or decrease must be approved by a doctor. The whole tablet should be swallowed after meals, and should not be chewed.

Adverse Reactions

In the early stage of medication, nitrate-induced vasodilatory headache may occur, as well as facial flushing, dizziness, orthostatic hypotension and reflex tachycardia. Occasionally, blood pressure may drop significantly, bradycardia and angina pectoris may worsen, and collapse and syncope may occur rarely.

Precautions

Acute circulatory failure (shock, circulatory collapse); severe hypotension (systolic blood pressure 90 mmHg); acute myocardial infarction with low filling pressure (unless under continuous hemodynamic monitoring); hypertrophic obstructive cardiomyopathy; constrictive pericarditis or cardiac tamponade; severe anemia; glaucoma; increased intracranial pressure; primary pulmonary hypertension; allergy to nitro compounds; allergy to isosorbide dinitrate or any ingredient of this product.

Special Population Medication

Precautions for children: All studies on this type of drug were conducted in adults. There is no data comparing the use of drugs in children and adults, so it is not recommended for use in children. Precautions for pregnancy and lactation: No toxic effects on embryos were observed in animal experiments on pregnant and lactating women, and it is not clear whether ISMN is excreted through breast milk. However, due to the lack of experience in the use of drugs in pregnant and lactating women, it should be used with caution. Precautions for the elderly: It is not yet clear.

Drug Interactions

The combination of this drug with other vasodilators, calcium antagonists, beta-receptor blockers, antihypertensive drugs, tricyclic antidepressants and alcohol can enhance the blood pressure lowering effect of this drug. It can also enhance the pressor effect of dihydroergot alkaloids. The simultaneous use of steroid anti-inflammatory drugs can reduce the efficacy of this drug.
