Chloroquine phosphate injection

Function and Efficacy

The mechanism of action of chloroquine is not yet very clear. It was once believed that chloroquine could insert into the double helix structure of malarial parasite DNA and form a complex with DNA, thereby preventing DNA replication and RNA transcription. Chloroquine can also inhibit the incorporation of phosphate into malarial parasite DNA and RNA, interfering with the reproduction of malarial parasites due to the reduction of nucleic acid synthesis. Chloroquine can bind tightly to soluble FP (ferroprotoporphyrin IX) in vitro, and only has a killing effect on erythrocytic malarial parasites that can produce hemochromatin, suggesting the importance of FP in the mechanism of action of chloroquine, thereby leading to the formation of a toxic chloroquine-FP complex and exerting its antimalarial effect. Chloroquine mainly acts on the erythrocytic schizonts of malarial parasites. After 48-72 hours, the schizonts in the blood are killed, thus controlling the clinical symptoms of malaria.

Ingredients

The chemical name of this product is: N', N'-diethyl-N4-(7-chloro-4-quinolyl)-1,4-pentanediamine diphosphate Molecular formula: C18H26ClN3·2H3PO4 Molecular weight: 515.87

Appearance

This product is a colorless or almost colorless clear liquid.

Indication

It is used to treat patients with malignant malaria, vivax malaria, malaria malariae and ovale malariae who are sensitive to chloroquine and cannot take it orally. It can also be used to treat patients with extraintestinal amebiasis such as amoebic liver abscess, and oral medication can be used after the condition improves.

Usage and Dosage

Patients with cerebral malaria should receive an intravenous drip of 18-24 mg/kg on the first day (60 kg is used for patients weighing more than 60 kg), 12 mg/kg on the second day, and 10 mg/kg on the third day. The concentration is 0.5 g of chloroquine phosphate added to 500 ml of 10% glucose solution or 5% glucose sodium chloride injection, and the intravenous drip rate is 12-20 drops per minute.

Adverse Reactions

There may be photophobia, impaired color vision, decreased visual acuity, and in severe cases blindness. It may also cause inhibition of the sinoatrial node, leading to arrhythmias, physical weakness, and in severe cases, Adams-Stokes syndrome and even death.

Precautions

Patients with liver and kidney dysfunction and heart disease are prohibited from using this product. Patients with severe erythema multiforme, porphyria, psoriasis and mental illness should use it with caution.

Special Population Medication

Precautions for children: The plasma clearance rate of newborns may be reduced and the serum concentration may increase, so it should be used with caution. This product is not recommended for children. Precautions for pregnancy and lactation: It is forbidden for pregnant and lactating women.

Drug Interactions

1. This product can easily cause allergic dermatitis when used together with phenylbutazone. 2. It can easily aggravate liver damage when used together with chlorpromazine. 3. This product has a direct inhibitory effect on neuromuscular junctions, and streptomycin can aggravate this side effect. 4. Patients with digitalization can easily cause heart block after using this product. 5. This product can increase the chance of bleeding when used together with heparin or penicillamine. 6. This product can cure vivax malaria when used together with primaquine. 7. When used together with ammonium chloride, it can accelerate excretion and reduce blood concentration. 8. When used together with monoamine oxidase inhibitors, it can increase toxicity. 9. When used together with flurandrenolide, it can easily cause exfoliative erythroderma. 10. When used together with chloroquine analogs (aminoquine, hydroxychloroquine, etc.), the blood concentration of chloroquine can be increased.
