Aprotinin Sodium Chloride Injection

Function and Efficacy

Aprotinin is a broad-spectrum protease inhibitor that can be used to regulate the body's inflammatory response (SIR) caused by cardiopulmonary bypass (CPB) surgery. SIR can activate related bleeding, fibrinolysis, and cellular and humoral inflammatory activation systems. Aprotinin can reduce inflammatory response, fibrinolysis, and thrombin production through its multiple regulatory inhibitory effects [such as vasodilators, plasmin]. Aprotinin can inhibit the release of inflammatory cytokines and maintain the balance of glycoproteins. Aprotinin can reduce the loss of platelet glycoproteins (such as GpⅠbGpⅡb/Ⅲa) and prevent the expression of inflammatory adhesive glycoproteins in granulocytes (such as CDⅡb). The role of aprotinin in CPB also includes reducing inflammatory response, which means that the need for allogeneic blood transfusion can be reduced and can

Appearance

This product is a colorless clear liquid.

Indication

Used to prevent and reduce intraoperative bleeding in patients undergoing coronary artery bypass grafting with extracorporeal circulation, and to reduce the need for blood transfusion accordingly.

Usage and Dosage

There are two recommended regimens, regimen A and regimen B. There is no difference in the efficacy of regimen A and regimen B in low-risk patients. Therefore, the dosage (A or B) depends only on the doctor's judgment. This product can be made into a solution of 5.6 units/ml, equivalent to 1.4mg/ml. This product must be injected into the central vein for intravenous injection. Remember not to inject other drugs into the same vein. Both regimens include a 1ml test dose, an initial dose, a dose added to the circulating fluid of cardiopulmonary bypass ("priming" dose) and a routine infusion volume. When this product is added to the circulating fluid, in order to avoid its physical incompatibility with heparin, it is necessary to add a reagent to dilute and mix with other ingredients. Routine A and Routine B (including 1ml test volume) are shown in the table below: 1

Adverse Reactions

In general, this product is well tolerated. Reports of adverse events are often related to cardiac surgery. Adverse reactions with an incidence of 1-2% have been reported: Systemic: sepsis, death, multiple system organ failure, immune system disorders, hemoperitoneum. Cardiovascular: congestive heart failure, bundle branch block, myocardial ischemia, heart block, pericardial effusion, ventricular arrhythmia (including ventricular tachycardia and ventricular fibrillation), shock, pulmonary hypertension. Digestive: indigestion, gastrointestinal bleeding, jaundice, liver failure. Vascular and lymphatic: arterial thrombosis (including pulmonary artery, coronary artery), pulmonary embolism, thrombophlebitis, deep thrombophlebitis, cerebrovascular accidents, cerebral embolism. Other blood diseases include leukocytosis, coagulation

Precautions

People who are allergic to this product.

Drug Interactions

This product is known to have antifibrinolytic activity and can therefore inhibit the effects of fibrinolysis. In a study of 9 untreated hypertensive patients, intravenous injection of 1112 units of this product blocked the acute lowering of blood pressure by 100 mg of carapril for more than two hours. In the presence of heparin, the diatomaceous earth surface activation method showed that this product can prolong the activated clotting time, but the activated clotting time of kaolin was not affected.

Storage

Keep in a dark, airtight container in a cool place.

Packaging Specification

100ml: 556 units of aprotinin and 0.9g of sodium chloride
