Isosorbide Mononitrate Tablets

Isosorbide mononitrate (ISMN) is the main biologically active metabolite of isosorbide dinitrate. Like other organic nitrates, its main pharmacological action is to relax vascular smooth muscle. ISMN releases nitric oxide (NO), which is the same as endothelial relaxing factor. It activates guanylate cyclase, increases cyclic guanosine monophosphate (cGMP) in smooth muscle cells, and thus relaxes vascular smooth muscle, dilates peripheral arteries and veins, and has a stronger dilating effect on veins. Venous dilation causes blood to remain in the periphery, reduces the amount of blood returning to the heart, and reduces left ventricular end-diastolic pressure and pulmonary capillary wedge pressure (preload). Arterial dilation reduces peripheral vascular resistance, systolic arterial pressure, and mean arterial pressure (afterload). Coronary artery dilation increases coronary perfusion. The overall effect is to reduce myocardial oxygen consumption, increase oxygen supply, and relieve angina pectoris.

The main chemical name of this product is: 1,4:3,6-dianhydro-D-sorbitol-5-mononitrate. Its molecular formula is: C6H9NO6, and its molecular weight is: 191.14.

This product is white tablets.

Long-term treatment of coronary heart disease, prevention of vasospastic and mixed angina pectoris, and also suitable for treatment after myocardial infarction and long-term treatment of chronic heart failure.

Tablets: 10-20 mg orally, 2 to 3 times a day. For severe cases, 40 mg can be used, 2 to 3 times a day.

Common adverse reactions are headaches (>10% of patients), which usually disappear after continued use. Hypotension and/or dizziness (1-10%) may occur at the beginning of treatment or when the dose is increased, accompanied by dizziness, drowsiness, emission tachycardia and fatigue. Occasionally (<1%), nausea, vomiting, facial flushing, skin allergies (such as rash), which may sometimes be severe, and rare exfoliative dermatitis. Severe hypotensive reactions have been reported for organic nitrates, including nausea, vomiting, restlessness, pallor and sweating. Rarely, collapse (sometimes accompanied by arrhythmias and syncope). Severe hypotension exacerbating angina pectoris is also rare. During treatment with Pandego (isosorbide mononitrate tablets), transient hypoxemia may occur due to redistribution of blood into the alveolar areas with insufficient ventilation. The phenomenon of stomach pain may be due to the fact that nitrates can relax the sphincter.

Acute circulatory failure (shock, circulatory collapse); severe hypotension (systolic blood pressure 90 mmHg); acute myocardial infarction with low filling pressures (unless under continuous hemodynamic monitoring); hypertrophic obstructive cardiomyopathy; constrictive pericarditis or cardiac tamponade; severe anaemia; glaucoma; increased intracranial pressure; allergy to nitro compounds.

Precautions for children: All studies on this type of drug were conducted in adults. There is no data comparing the use of drugs in children and adults, so it is not recommended for use in children. Precautions for pregnancy and lactation: No toxic effects on embryos were observed in animal experiments, and it is not clear whether ISMN is excreted through breast milk. However, due to the lack of experience in the use of drugs in pregnant and lactating women, it should be used with caution. Precautions for the elderly: Elderly patients may be more sensitive to this type of drug and are more likely to experience reactions such as dizziness.

Combination with other vasodilators, calcium antagonists, beta-receptor blockers, antihypertensive drugs, tricyclic antidepressants and alcohol can enhance the blood pressure lowering effect of this class of drugs.
