Doxazosin Mesylate Tablets

Doxazosin is a selective α-receptor blocker. It achieves the effects of vasodilation, reducing vascular resistance, and lowering blood pressure by selectively and competitively blocking postganglionic α1-adrenergic receptors. Doxazosin is an effective blocker of the A1 subtype of α1-adrenergic receptors, and more than 70% of prostate α1 receptors are A1 subtypes. Therefore, it can improve the symptoms of benign prostatic hyperplasia by selectively blocking α1-adrenergic receptors (mainly A1 subtypes) located in the matrix, capsule, and bladder neck smooth muscle. Doxazosin can moderately increase the ratio of high-density lipoprotein cholesterol to total cholesterol, but its clinical significance has not yet been determined. In vitro studies have shown that 5 μmol of doxazosin 6,- and 7,-hydroxylated metabolites have antioxidant effects.

For patients with primary mild to moderate hypertension whose blood pressure is difficult to control with medication alone, it can be used in combination with diuretics, beta-blockers, calcium antagonists or angiotensin-converting enzyme inhibitors (ACEⅠ).

The initial dose is 1 mg (1 tablet) to reduce the incidence of orthostatic hypotension and syncope with the first dose. Orthostatic hypotension often occurs between 2-6 hours after taking the drug, so blood pressure should be measured during the first dose and each time the dose is increased. If the drug is discontinued for several days, the drug should be restarted according to the initial treatment plan. The initial dose is 1 mg (1 tablet), once a day. Depending on the patient's orthostatic blood pressure response (based on the measured values 2-6 hours and 24 hours after taking the drug), the dose can be increased to 2 mg (2 tablets) once a day, and then increased to 4 mg (4 tablets) once a day as needed, and then 6 mg (6 tablets) once a day to achieve the ideal antihypertensive effect. In controlled clinical trials, the most common adverse reaction was postural hypotension (rarely accompanied by syncope), and non-specific adverse reactions included: dizziness, fatigue, hypotension, peripheral edema, dyspnea, headache, malaise, postural dizziness, vertigo, weakness, drowsiness, gastrointestinal reactions (abdominal pain, diarrhea, nausea, vomiting, gastroenteritis), dry mouth, back pain, chest pain, palpitations, tachycardia, myalgia, bronchitis, cough, itching, urinary incontinence, cystitis and rhinitis. Irritability and tremor (rare cases) have been reported. Occasionally, there have been reports of priapism and impotence associated with alpha-1 blockers, including doxazosin. There have also been reports of drug allergic reactions, rash, thrombocytopenia, purpura, epistaxis, leukopenia, hematuria, cholestasis, jaundice, and abnormal liver function tests.

Patients with known hypersensitivity to quinazolines (such as prazosin and terazosin) or any of the ingredients of this product are contraindicated. Patients with a recent myocardial infarction are contraindicated. Patients who have been treated with doxazosin and have a myocardial infarction should have their post-infarction treatment decided on an individual basis. Patients with a history of gastrointestinal obstruction, esophageal obstruction, or any degree of gastrointestinal tract stenosis are contraindicated.

Precautions for children: As an antihypertensive drug, the safety and effectiveness of this product in children have not yet been determined. Precautions for pregnancy and lactation: The safety of this product for pregnant women has not yet been determined. This product can only be used in pregnant women when it is absolutely necessary. It is not yet known whether this drug can be secreted through human breast milk. Lactating women should use this drug with caution. Precautions for the elderly: This product may cause obvious hypotension reactions in elderly hypertensive patients, and the daily maintenance dose must be reduced. In the treatment of benign prostatic hyperplasia, the safety and effectiveness of this product are consistent in the elderly and non-elderly.

Most (98%) doxazosin in plasma is bound to proteins. In vitro data from human plasma studies showed that doxazosin had no effect on the protein binding of digoxin, warfarin, phenytoin, and indomethacin. In clinical medication, no adverse drug interactions were found when doxazosin was used in combination with thiazide diuretics, furosemide, beta-blockers, nonsteroidal anti-inflammatory drugs, antibiotics, oral hypoglycemic drugs, uricosuric drugs, or anticoagulants.

Keep in a light-proof and sealed container. The validity period is tentatively set at two and a half years.
