Nimodipine Tablets
Function and Efficacy
Nimodipine is a 1,4-dihydropyridine calcium channel antagonist. Its pharmacological properties are to effectively regulate the level of intracellular calcium to maintain normal physiological functions. It has a particularly prominent effect on cerebral blood vessels and can bind to specific receptors of the central nervous system. This specificity enables this product to effectively prevent and treat ischemic damage to brain tissue caused by cerebral vasospasm caused by subarachnoid hemorrhage, inhibit the influx of extracellular calcium ions in vascular smooth muscle cells, and have a dilating effect on cerebral arteries in vitro or in vivo, normal or ischemic cerebral arteries. At an appropriate dose, it selectively dilates cerebral blood vessels and has almost no effect on peripheral blood vessels, but increasing the dose also has a good therapeutic effect on hypertension.

Indication
It is suitable for improving blood circulation in the recovery period of cerebral vasospasm after subarachnoid hemorrhage of various reasons and acute cerebrovascular disease.
Usage and Dosage
1. Ischemic cerebrovascular disease: Oral. 30-120 mg per day, divided into 3 times, for one month. 2. Migraine: Oral. 40 mg per time, 3 times per day, 12 weeks as a course of treatment, the effective rate is 88%, about half of the cases can be basically cured or significantly effective, it can reduce the pain degree, frequency and duration of attacks, and prevent the appearance of premonitory symptoms for vascular, tension, cluster and mixed headaches. 3. Cerebral vasospasm caused by subarachnoid hemorrhage: Oral. 40-60 mg per time, 3-4 times per day, 3-4 weeks as a course of treatment. For patients who need surgery, stop taking the medicine on the day of surgery, and continue to take it later. 4. Sudden deafness: Oral. 40-60 mg per day, divided into 3 times, 5 days as a course of treatment, generally 3-4 courses of treatment. 5. Mild and moderate hypertension: patients with hypertension and cerebrovascular disease can be given priority. Oral. Start with 40 mg once, 3 times a day, the maximum daily dose is 0.24 g.
Adverse Reactions
A large number of clinical practices have shown that when patients with subarachnoid hemorrhage are treated with nimodipine, about 11.2% of patients experience adverse reactions. The most common adverse reactions are: (1) Decreased blood pressure, the degree of which is related to the drug dose. (2) Hepatitis. (3) Skin irritation. (4) Gastrointestinal bleeding. (5) Thrombocytopenia. (6) Occasionally, transient dizziness, headache, facial flushing, vomiting, gastrointestinal discomfort, etc. In addition, after oral administration of nimodipine, some patients may experience elevated levels of alkaline phosphatase (ALP), lactate dehydrogenase (LDH), AKP, elevated blood sugar, and elevated platelet count in some individuals.

Special Population Medication
Precautions for children: Not yet clear. Precautions for pregnancy and lactation: 1. The drug can be secreted by breast milk, so it is not suitable for lactating women. 2. Animal experiments indicate that this product is teratogenic. Precautions for the elderly: Not yet clear.
Drug Interactions
(1) When used in combination with other calcium ion antagonists that act on the cardiovascular system, it can increase the effectiveness of other calcium ion antagonists. (2) When nimodipine 90 mg/day is used in combination with cimetidine 1000 mg/day for more than one week, the blood concentration of nimodipine can increase by 50%, which may be related to the inhibition of nimodipine metabolism by cimetidine in the liver cytochrome P450.
