Appearance
This product is a yellow tablet.
Indication
This product is mainly used to treat edema diseases, including congestive heart failure, ascites due to liver cirrhosis, nephrotic syndrome, etc. As well as water and sodium retention during adrenal glucocorticoid treatment, the main purpose is to correct the secondary aldosterone secretion increase in the above situation, and antagonize the potassium excretion effect of other diuretics. It can also be used to treat idiopathic edema.
Usage and Dosage
1Usual oral dose for adults. Start with 25~100mg per day, divided into 2 doses. The dose can be reduced when used with other diuretics. It can be changed to alternate-day therapy during the maintenance phase. The maximum dose does not exceed 300mg per day. 2Usual oral dose for children. Start with 2~4mg/kg per day based on body weight or 120mg/m2 per body surface area, divided into 2 doses, daily or alternate-day therapy. Adjust the dose as appropriate later. The maximum dose does not exceed 6mg/kg or 300mg/m2 per day.
Adverse Reactions
1 Common symptoms include hyperkalemia. 2 Rare symptoms include: ① Gastrointestinal reactions, such as nausea, vomiting, stomach cramps and diarrhea; ② Hyponatremia; ③ Dizziness and headache; ④ Photosensitivity. 3 Rare symptoms include: ① Allergies, such as rash and dyspnea; ② Blood system damage, such as granulocytopenia or even agranulocytosis, thrombocytopenic purpura, and megaloblastic anemia (interference with folic acid metabolism); ③ Kidney stones. It has been reported that the incidence of kidney stones in long-term users of this drug is 1/1500. The mechanism may be that the drug and its metabolites are oversaturated in urine, precipitate crystals and bind to the protein matrix, thus forming kidney stones.
Precautions
Contraindicated in case of hyperkalemia.
Special Population Medication
Precautions during pregnancy and lactation: 1. Animal experiments show that this drug can pass through the placenta, but the situation in humans is still unclear. 2. Experiments in cows show that this drug can be secreted in breast milk, but the situation in humans is still unclear. Precautions for the elderly: Elderly people are more likely to develop hyperkalemia and kidney damage when using this drug.
Drug Interactions
(1) Adrenal cortical hormones, especially those with strong mineralocorticoid effects, target=_blank adrenocorticotropic hormone can weaken the diuretic effect of this drug and antagonize its potassium-retaining effect. (2) target=_blank Estrogen can cause water and sodium retention, thereby weakening the diuretic effect of this drug. (3) Non-steroidal anti-inflammatory analgesics, especially indomethacin, can reduce the diuretic effect of this drug, and increase renal toxicity when used in combination. (4) Sympathomimetic drugs reduce the antihypertensive effect of this drug. (5) Dopamine enhances the diuretic effect of this drug. (6) When used in combination with drugs that cause a decrease in blood pressure, both the diuretic and antihypertensive effects are enhanced. (7) When used in combination with the following drugs, the chance of target=_blank hyperkalemia increases, such as potassium-containing drugs, stored blood (containing 30mmol/L potassium, and potassium up to 65mmol/L if stored for more than 10 days), angiotensin converting enzyme inhibitors, target=_blank angiotensin II receptor antagonists and target=_blank cyclosporine A. (8) When used in combination with glucose insulin solution, alkaline agents, and sodium-type potassium-lowering exchange resins, the chance of hyperkalemia decreases. (9) This drug prolongs the half-life of digoxin. (10) When used in combination with ammonium chloride, target=_blank metabolic acidosis is likely to occur. (11) When used in combination with nephrotoxic drugs, nephrotoxicity increases. (12) Sodium carbenoxolone and licorice preparations have aldosterone-like effects and can reduce the diuretic effect of this drug. (13) Because it can increase target=_blank blood uric acid, it can further increase target=_blank blood uric acid when used in combination with thiazide and loop diuretics, so it should be used in combination with drugs for the treatment of target=_blank gout. (14) It can increase blood sugar, so when used in combination with hypoglycemic drugs, the latter's dose should be appropriately increased.
