Function and Efficacy

This product is a carbonic anhydrase inhibitor, which can inhibit the production of aqueous humor and reduce intraocular pressure. The ease of aqueous humor outflow does not change. Acetazolamide can inhibit the activity of carbonic anhydrase in the ciliary epithelium, thereby reducing the production of aqueous humor (50% to 60%) and lowering intraocular pressure.

Indication

Suitable for treating various types of glaucoma

Usage and Dosage

Common dosage for adults: (1) For open-angle glaucoma, take 250 mg (1 tablet) orally for the first time, 1 to 3 times a day. The maintenance dose should be determined according to the patient's response to the drug. Try to use a smaller dose to control intraocular pressure; generally, 250 mg (1 tablet) twice a day can keep intraocular pressure within the normal range. (2) For secondary glaucoma and preoperative intraocular pressure reduction, take 250 mg (1 tablet) orally, once every 4 to 8 hours, generally 2 to 3 times a day. (3) For acute cases, double the initial dose to 500 mg (2 tablets), and then use 125 to 250 mg (0.5 to 1 tablet) for maintenance, 2 to 3 times a day.

Adverse Reactions

Common adverse reactions after taking the drug include: (1) numbness and tingling in the limbs; (2) general malaise syndrome: fatigue, weight loss, drowsiness, depression, lethargy, decreased libido, etc.; (3) gastrointestinal reactions: metallic taste, nausea, loss of appetite, indigestion, diarrhea; (4) renal reactions: polyuria, nocturia, kidney and urinary tract stones, etc. (5) temporary myopia may occur, and sulfonamide rash and exfoliative dermatitis may also occur. Rare side effects: (1) electrolyte imbalance: metabolic acidosis, hypokalemia, supplementation with sodium bicarbonate and potassium salt may alleviate symptoms; (2) hearing loss; (3) the most serious adverse reactions are hematopoietic system disorders: acute hemolytic anemia, granulocytopenia, thrombocytopenia, eosinophilia, aplastic anemia, and renal failure. Long-term use of the drug may aggravate symptoms such as hypokalemia, hyponatremia, electrolyte imbalance and metabolic acidosis. Since the decrease in blood potassium can weaken the intraocular pressure-lowering effect of this product, this product can induce or aggravate the condition of patients with kidney stones. If renal colic and hematuria occur, the drug should be discontinued immediately.

Precautions

Hepatic and renal dysfunction can cause hyponatremia, hypokalemia, hyperchloremic acidosis, adrenal failure and adrenocortical insufficiency (Addison's disease), and hepatic coma.

Special Population Medication

Precautions for children: Anti-glaucoma, 2 to 3 times a day, 5 to 10 mg/kg orally each time according to body weight, or 300 to 900 mg/m2 orally according to body surface area per day, taken in 2 to 3 times. Precautions for pregnancy and lactation: Animal experiments have confirmed that the use of acetazolamide at a dose 10 times higher than that of adults has a higher incidence of teratogenicity in rodent fetuses, so its pros and cons must be considered. Reports have pointed out that women who are about to give birth and pregnant women should not use it, especially in the first 3 months of pregnancy. Breastfeeding women who really need to use this product should suspend breastfeeding. Precautions for the elderly: Not yet clear.

Drug Interactions

(1) When used in combination with adrenocorticotropic hormone, glucocorticoids, and especially mineralocorticoids, it can cause severe hypokalemia. When using these drugs together, it is important to monitor serum potassium concentration and cardiac function. It should also be estimated that long-term simultaneous use may increase the risk of hypocalcemia and cause osteoporosis, because these drugs can increase calcium excretion. (2) When used in combination with amphetamines, anticholinergics, and especially atropine and quinidine, the excretion of this product is reduced due to the formation of alkaline urine, which may aggravate or prolong adverse reactions. (3) When used in combination with antidiabetic drugs (such as insulin), it can reduce hypoglycemia reactions, because this product can cause hyperglycemia and glycosuria, so the dose should be adjusted. (4) When used in combination with phenobarbital, carbamazepine, or phenytoin, it may cause an increase in the incidence of osteomalacia. (5) When digitalis glycosides are used in combination with this product, the toxicity of digitalis may be increased and hypokalemia may occur. (6) When used in combination with mannitol or urea, it can increase urine volume while enhancing the effect of lowering intraocular pressure.
