                                                              1. Chemical Structure: Irinotecan hydrochloride, with the chemical formula C33H38N4O6•HCl, presents a complex molecular structure.

                                                              2. Pharmacological Profile: Irinotecan hydrochloride, a topoisomerase I inhibitor, exhibits potent anti-cancer properties.

                                                              3. Therapeutic Action: Irinotecan hydrochloride exerts its therapeutic effects by interfering with DNA replication and inducing apoptosis in cancer cells.

                                                              4. Clinical Applications: Irinotecan hydrochloride finds utility in the treatment of various malignancies, including colorectal, gastric, and lung cancers.

                                                              5. Chemical Properties: Irinotecan hydrochloride typically appears as a white to yellowish crystalline powder, soluble in water and dimethyl sulfoxide (DMSO).

                                                              6. Pharmacokinetics: Following administration, Irinotecan hydrochloride undergoes extensive metabolism in the liver, yielding active and inactive metabolites.

                                                              7. Adverse Effects: Common side effects associated with Irinotecan hydrochloride include diarrhea, myelosuppression, nausea, and vomiting.

                                                              8. Dosage Forms: Irinotecan hydrochloride is commercially available in injectable formulations for intravenous administration.

                                                              9. Drug Interactions: Irinotecan hydrochloride may interact with other medications, necessitating dose adjustments or careful monitoring.

                                                              10. Clinical Trials: Ongoing research continues to explore the efficacy and safety of Irinotecan hydrochloride in various cancer types and treatment regimens.

                                                              11. Resistance Mechanisms: Resistance to Irinotecan hydrochloride can arise due to alterations in drug metabolism, efflux mechanisms, or DNA repair pathways.

                                                              12. Combination Therapies: Irinotecan hydrochloride is often used in combination with other chemotherapy agents or targeted therapies to enhance treatment outcomes.

                                                              13. Patient Management: Close monitoring of patients receiving Irinotecan hydrochloride is essential to manage adverse effects and optimize therapeutic response.

                                                              14. Special Populations: Dosage adjustments may be necessary for patients with hepatic or renal impairment to prevent toxicity or ensure adequate efficacy.

                                                              15. Future Directions: Ongoing research aims to elucidate novel formulations or combination strategies to improve the therapeutic index of Irinotecan hydrochloride and expand its clinical utility.

Irinotecan hydrochloride (anhydrous) is a hydrochloride obtained by combining irinotecan with one molar equivalent of hydrochloric acid. Used (in the form of its trihydrate) in combination with fluorouracil and leucovorin, for the treatment of patients with metastatic adenocarcinoma of the pancreas after disease progression following gemcitabine-based therapy. It is converted via hydrolysis of the carbamate linkage to its active metabolite, SN-38, which is ~1000 times more active. It has a role as an EC 5.99.1.2 (DNA topoisomerase) inhibitor, an antineoplastic agent, an apoptosis inducer and a prodrug. It contains an irinotecan(1+).|Irinotecan and topotecan are semisynthetic derivatives of the plant alkaloid camptothecin and are used as antineoplastic agents in the therapy of colorectal, ovarian and non-small cell lung cancer. Both irinotecan and topotecan are associated with an appreciable rate of serum enzyme elevations during therapy, and irinotecan has been implicated in causing steatohepatitis when given as cyclic anticancer therapy.|A semisynthetic camptothecin derivative that inhibits DNA TOPOISOMERASE I to prevent nucleic acid synthesis during S PHASE. It is used as an antineoplastic agent for the treatment of COLORECTAL NEOPLASMS and PANCREATIC NEOPLASMS.
