Introduction	It belongs to the first generation of tretinoin and is the stereoisomer of all trans tretinoin. Oral administration has anti lipid effect and has special curative effect on severe acne. The mechanism of action is as follows: it can reduce the sebaceous glands, inhibit the activity of sebaceous glands, and reduce the secretion of sebaceous glands, so as to inhibit the reproduction of Propionibacterium acnes which depends on the growth of lipid environment. It has the effect of anti keratinization, inhibiting the excessive proliferation of epidermal cells and promoting their differentiation, and can reduce the keratinization of hair follicle mouth and sebaceous duct. It can also affect the function of monocytes and lymphocytes, inhibit the chemotaxis of neutrophils and has anti-inflammatory activity. It selectively binds to retinoic acid chemicalbook nuclear receptor and plays a therapeutic role. It can inhibit the proliferation of Corynebacterium acnes. It has the effect of immunostimulation at low dose and immunosuppression at high dose. It can inhibit the formation of collagenase and gelatinase in skin, and inhibit the activity of ornithine decarboxylase. The induction of ornithine decarboxylase can play a major role in skin mutation, so Accutane can also inhibit the occurrence of tumor. The mechanism of action of this product is similar to that of retinoic acid. It can induce the proliferation of epidermal cells, promote the differentiation of epidermal granular layer into stratum corneum, regulate the process of abnormal keratinization of hair follicle sebaceous epith elium, and play a therapeutic role.
