Adapalene is a topical retinoid medication primarily used for the treatment of mild-to-severe acne vulgaris. It is available in various forms, including creams, gels, lotions, solutions and swabs.

Mechanism of Action

Adapalene works by binding to retinoic acid receptors (RARs) in skin cells. This binding leads to several effects:
Modulation of Cellular Processes: Adapalene modulates cellular differentiation, keratinization, and inflammatory processes, which are key factors in acne pathology.
Comedolytic and Anticomedogenic Action: By normalizing keratinocyte differentiation, adapalene reduces microcomedone formation, unclogs pores, and increases exfoliation through increased cell turnover.
Anti-Inflammatory Effects: Adapalene reduces inflammation by down-regulating toll-like receptor 2 (TLR-2) expression and inhibiting the transcription factor activator protein 1 (AP-1). This action helps to reduce inflammation-mediated acne symptoms.
Adapalene's stable chemical structure allows it to be used with benzoyl peroxide without losing efficacy.

Applications

Adapalene is a first-line therapy in acne treatment and can be used independently or with benzoyl peroxide and/or an antimicrobial agent for maximum efficacy. It increases the efficacy and penetration of other topical acne medications and hastens the improvement of postinflammatory hyperpigmentation caused by acne. Adapalene can also be used as maintenance therapy in the long term.
Adapalene can also inhibit keratinocyte differentiation and decrease keratin deposition, making it an effective treatment for keratosis pilaris and callus. It has also been reported for the treatment of warts, molluscum contagiosum, Darier's disease, photoaging, pigmentary disorders, actinic keratoses, and alopecia areata, although these are not FDA-approved indications.

Routes of Administration

Adapalene is administered topically, applied directly to the affected skin areas.

Safety and Side Effects

Common side effects of adapalene include skin irritation such as redness, itching, burning, or stinging, as well as dry, flaky skin. These side effects usually occur during the first month of therapy and decrease thereafter. Less common side effects include acne flare, contact dermatitis, eczema, and rash. Serious allergic reactions, though less common, can occur. Symptoms of a serious allergic reaction include breathing problems or wheezing, racing heart, fever or general ill feeling, and swelling.

Pharmacokinetics

Absorption: Absorption of adapalene through the skin is low. Studies have found very low levels of the drug in the blood plasma of patients using adapalene topically.
Distribution: Adapalene is highly lipophilic and readily penetrates hair follicles.
Pharmacodynamics: Adapalene binds to nuclear retinoic acid receptors, modulating gene transcription and leading to normalized keratinocyte differentiation and decreased inflammation.
