Description	Voriconazole is a triazole antifungal medication that is generally used to treat serious, invasive fungal infections. Voriconazole has also been proven effective against a number of other serious fungal pathogens. This includes infections by Fusarium spp and Scedosporium apiospermum (asexual form of Pseudallescheria boydii). Although infrequently seen, these molds are emerging as more common and deadly causes of fungal infection in seriously immunocompromised patients, and the development of voriconazole has been an important advance in their treatment as they are generally resistant to other antifungal agents (including amphotericin B). Voriconazole is the first and only drug ever specifically indicated for their treatment by the FDA. Voriconazole has also been used to treat severe fungal corneal infection.

Applications

Voriconazole is used to treat invasive fungal infections such as invasive aspergillosis, candidemia in non-neutropenic patients, esophageal candidiasis, serious fungal infections caused by Scedosporium and Fusarium species, and other severe fungal infections in immunocompromised patients.

Safety and Side Effects

Common adverse effects include visual disturbances (e.g., blurred vision, altered color perception), hepatotoxicity, rash, photosensitivity, and gastrointestinal symptoms. Serious but less common effects include liver enzyme elevations, QT prolongation, and central nervous system effects such as hallucinations. Voriconazole is known for hepatotoxicity and photosensitivity reactions, requiring monitoring during therapy.

Routes of Administration

Voriconazole is available as oral tablets, oral suspension, and intravenous infusion, allowing flexible administration depending on patient condition.

Pharmacokinetics

Absorption: Voriconazole has high oral bioavailability with peak plasma concentrations reached within 1 to 2 hours. Its absorption is not significantly affected by gastric pH modifiers like ranitidine or omeprazole.
Distribution: It has a large volume of distribution (~4.6 L/kg), indicating extensive tissue penetration, and about 58% plasma protein binding.
Metabolism: Voriconazole undergoes extensive hepatic metabolism primarily via cytochrome P450 enzymes CYP2C19, CYP2C9, and CYP3A4. CYP2C19 plays a major role and exhibits genetic polymorphism affecting drug levels. The main circulating metabolite is voriconazole N-oxide, which is inactive.
Elimination: Less than 2% of the drug is excreted unchanged in urine. The drug follows nonlinear (saturable) pharmacokinetics, with dose-dependent half-life ranging from 6 to 24 hours. Clearance varies widely among individuals, averaging 5.25–7 L/h in healthy adults.
Drug Interactions: Voriconazole inhibits CYP2C19, CYP2C9, and CYP3A4, increasing plasma concentrations of many drugs metabolized by these enzymes. It is contraindicated with drugs like sirolimus due to significant interaction risks.
