Trimethoprim is an antifolate antibacterial agent that selectively inhibits bacterial dihydrofolate reductase (DHFR), an essential enzyme responsible for converting dihydrofolic acid (DHF) to tetrahydrofolic acid (THF). THF is a critical precursor for the synthesis of bacterial nucleic acids and proteins, so inhibition of DHFR by trimethoprim disrupts DNA synthesis and bacterial replication, ultimately leading to bacteriostatic effects when used alone.
When combined with sulfamethoxazole, which inhibits an earlier step in folate synthesis (dihydropteroate synthase), trimethoprim exhibits synergistic and bactericidal activity by blocking two consecutive steps in the bacterial folate pathway.

Applications

Primarily used to treat acute uncomplicated urinary tract infections caused by susceptible bacteria such as Escherichia coli, Klebsiella pneumoniae, Enterobacter spp., Proteus mirabilis, and coagulase-negative Staphylococcus species.
Used in combination with sulfamethoxazole for a broader range of infections including acute otitis media in children, acute exacerbations of chronic bronchitis in adults, enteritis caused by Shigella, prophylaxis and treatment of Pneumocystis jiroveci pneumonia, and travelers’ diarrhea caused by enterotoxigenic E. coli.
Occasionally used as monotherapy for urinary tract infections and prophylaxis in certain clinical scenarios.

Safety and Side Effects

Common: Rash, pruritus, nausea, vomiting, diarrhea, headache, and mild gastrointestinal disturbances.
Serious but rare: Severe allergic reactions including angioedema, Stevens-Johnson syndrome, toxic epidermal necrolysis, blood disorders such as thrombocytopenia, leukopenia, neutropenia, aplastic anemia, and hemolytic anemia.
Metabolic: Hyperkalemia is relatively common, especially in elderly or patients with renal impairment or HIV infection.
Other: Antibiotic-associated diarrhea including Clostridioides difficile infection can occur, sometimes weeks after treatment.
Patients should seek immediate medical attention if symptoms of severe allergic reactions, blood disorders, or persistent diarrhea occur.

Pharmacokinetics

Absorption: Rapidly absorbed orally with peak serum concentrations (~1 µg/mL) reached within 1 to 4 hours after a 100 mg dose. Steady-state concentrations are typically achieved after about 3 days of repeated dosing.
Distribution: Widely distributed into tissues including sputum, bronchial secretions, middle ear fluid, vaginal fluids (concentrations ~1.6 times serum levels), crosses the placenta, and is excreted in breast milk.
Protein Binding: Approximately 44% bound to plasma proteins.
Metabolism: Undergoes oxidative metabolism mainly via CYP2C9 and CYP3A4 enzymes, producing demethylated metabolites; the parent drug remains the active form.
Elimination: About 50–60% excreted unchanged in urine within 24 hours, with the remainder metabolized in the liver.
Half-life: Approximately 8–10 hours, supporting twice-daily dosing.
