Source: Extracted from Cephalotaxus fortunei or related species within the Cephalotaxaceae family, homoharringtonine (HHT) is a self-developed anticancer drug in China.
Chemical Properties: Molecular formula: C₂₉H₃₉NO₉; molecular weight: 545.62. It appears as a white or light yellow amorphous powder, slightly soluble in water, and readily soluble in organic solvents such as methanol and ethanol.
Mechanism of Action:
- Inhibition of Protein Synthesis: Competes with aminoacyl-tRNA for the ribosomal A site, blocking initiation and elongation of protein synthesis.
- Induction of Apoptosis: Regulates alternative splicing of BCL-x and Caspase 9, promoting cancer cell apoptosis.
- Inhibition of Proliferation: Disrupts differentiation and proliferation signaling pathways in tumor cells.

II. Clinical Applications

Acute Leukemia:
- Acute Myeloid Leukemia (AML): Effective against various subtypes, including acute promyelocytic leukemia (APL).
- Acute Lymphoblastic Leukemia (ALL): Partial efficacy, with weaker responses compared to AML.
Myelodysplastic Syndromes (MDS): Improves hematological and bone marrow parameters in some patients.
Chronic Myeloid Leukemia (CML):
- Used in patients resistant or intolerant to tyrosine kinase inhibitors (TKIs).

Omacetaxine is associated with a low rate of serum enzyme elevation during therapy, but has not been linked to cases of clinically apparent liver injury with jaundice.
