    Description

Trametinib, also known as GSK1120212, is a n orally bioavailable inhibitor of mitogen-activated protein kinase kinase (MEK MAPK/ERK kinase) with potential antineoplastic activity. Trametinib specifically binds to and inhibits MEK 1 and 2, resulting in an inhibition of growth factor-mediated cell signaling and cellular proliferation in various cancers. MEK 1 and 2, dual specificity threonine/tyrosine kinases often upregulated in various cancer cell types, play a key role in the activation of the RAS/RAF/MEK/ERK signaling pathway that regulates cell growth. On May 29, 2013, FDA approved T rametinib.

    Trametinib More Info

Trametinib is an oral, small-molecule, selective inhibitor of MEK1 and MEK2, which are key components of the MAPK signaling pathway. This pathway is often dysregulated in cancer, leading to uncontrolled cell proliferation and survival. Trametinib works by inhibiting the activation and kinase activity of MEK1 and MEK2, thereby reducing cell proliferation, causing G1 cell cycle arrest, and inducing apoptosis.

    Mechanism of Action

Trametinib specifically binds to MEK1 and MEK2, preventing their activation and thus inhibiting the downstream ERK signaling pathway. This action helps to slow down the growth and spread of cancer cells. The specificity of trametinib for MEK1/2 has been confirmed against a panel of over 180 kinases, including B-Raf, C-Raf, and the closest kinase homolog MEK5. In vitro studies have shown that trametinib decreases cell proliferation, causes G1 cell-cycle arrest, and induces apoptosis.

    Clinical Outcomes

Trametinib was first approved by the FDA in 2013 for the treatment of unresectable or metastatic BRAF V600E or V600K mutation-positive melanoma. In a phase III clinical trial, trametinib demonstrated significantly improved response rates (22% vs. 8%), median progression-free survival (4.8 months vs. 1.5 months), and 6-month overall survival (81% vs. 67%) compared with chemotherapy.

When used in combination with dabrafenib, a BRAF inhibitor, trametinib has shown even better clinical outcomes. The combination therapy has resulted in higher response rates, prolonged progression-free survival, and overall survival compared to BRAF inhibitors alone. For example, in the COMBI-d trial, the combination of dabrafenib and trametinib significantly prolonged progression-free survival (9.3 months vs. 8.8 months) and improved the objective response rate (67% vs. 51%) compared to dabrafenib alone.

    Pharmacokinetics

Trametinib is rapidly absorbed, with the median time to maximum plasma concentration occurring 1.5 hours after a single oral administration under fasting conditions. The absolute oral bioavailability of a single 2.0 mg tablet is moderate to high (72%) relative to a coadministered intravenous microdose. The drug accumulates with repeat dosing, with a mean accumulation ratio of 5.97 at the recommended dose of 2 mg once daily and a terminal half-life of 5.3 days.

    Adverse Effects

Common side effects of trametinib include rash, diarrhea, fatigue, peripheral edema, nausea, and acneiform dermatitis. When used in combination with dabrafenib, additional common side effects may include fever, chills, headache, vomiting, and arthralgia. The combination therapy also has a lower incidence of cutaneous squamous cell carcinoma compared to BRAF inhibitor monotherapy.

    Future Directions

Trametinib is currently being explored in other subsets of melanoma and other malignancies, such as non-small cell lung cancer, pancreatic cancer, and hepatobiliary cancers. Improved understanding of predictive biomarkers of response and resistance is needed to identify appropriate patients most likely to benefit from MEK inhibitor-based regimens.

    ECHEMI Editorial Reference
Trametinib is a potent MEK inhibitor that inhibits MEK1 and MEK2 with IC50s of about 2 nM. Due to the poor solubility of Trametinib, Trametinib DMSO solvate (Cat. No.: HY-10999A) is recommeded.
