Ribociclib , also known as LEE-011, is a potent inhibitor of cyclin-dependent kinases 4 and 6 (CDK4/6). Developed by Novartis Pharmaceuticals Ltd., this drug targets the cyclin D1/CDK4 and cyclin D3/CDK6 cellular pathways, exhibiting significant antitumor activity. Ribociclib binds to cyclin proteins, facilitating the transition of cell cycle phases, initiating DNA synthesis, and regulating cellular transcription.

Efficacy

Cell cycle protein-dependent kinases (CDKs) 4 and 6 are enzymes that have been shown to promote cell division and proliferation in both normal and cancer cells. Many cancer cells show abnormalities that increase CDK activity and lead to inactivation of certain tumor suppressor genes. In combination with other drugs, such as ALK or MEK inhibitors, ribociclib has been shown to act synergistically, resulting in an improved response. Again, this may be the result of “crosstalk” between signaling pathways. Simply blocking one pathway in cancer tumorigenesis can sometimes lead to “tumor compensation,” in which the tumor compensates for the blocked signaling pathway by using other pathways to survive. By simultaneously blocking several pathways, tumors are thought to be less able to compensate and a greater anti-tumor response is often observed. Combining ribosomes with other drugs has been shown to reduce resistance to these drugs. In other words, the development of drug resistance in cancer can be mitigated by adding ribosomes to the treatment regimen.

Applications

LEE011 (Reboxetil) is an inhibitor of the cell cycle proteins D1 / CDK4 and CDK6 and is used to treat certain types of breast cancer. It is also being studied as a treatment for other drug-resistant cancers.

In vitro researches

LEE011, a dual CDK4/CDK6 inhibitor, significantly inhibited the growth of 12 out of 17 neuroblastomas with a mean IC50 value of 307 nM.Inhibition of neuroblastomas is primarily an inhibition of cell growth, which is regulated by G1 cell cycle block and cellular senescence.

In vivo studies

LEE011 (200 mg/kg daily, oral) caused a significant growth delay in BE2C or 1643 cells in mice without weight loss or other signs of toxicity.

Side Effects

The most common side effects studied with LEE011 were lower blood counts, primarily neutropenia (in 75% of patients compared to 5% on placebo), and anemia (18% vs. 5%). Gastrointestinal disorders were also common, such as nausea (52% vs. 29%) and diarrhea (35% vs. 22%), and hair loss (33% vs. 16%). The drug also increases QT time and liver enzymes (alanine aminotransferase, aspartate aminotransferase).
