Nystatin is a polyene antifungal antibiotic produced by the bacterium Streptomyces noursei, with the CAS Registry Number 1400-61-9. Its molecular structure consists of a large macrocyclic lactone ring, a hallmark of polyene antifungals, featuring a conjugated system of multiple double bonds and a mycosamine sugar moiety. This specific amphipathic structure—with a hydrophilic polyol region and a hydrophobic polyene chain—is essential for its antifungal activity, allowing it to bind to and disrupt fungal cell membranes. It is a yellow to light tan powder with a characteristic cereal-like odor, photosensitive, and poorly soluble in water.

The antifungal mechanism of nystatin is primarily fungicidal and involves a specific interaction with ergosterol, the principal sterol component of fungal cell membranes. Nystatin molecules bind to ergosterol in the membrane, forming transmembrane pores or channels. These aggregates disrupt the osmotic integrity of the membrane, causing essential intracellular components like potassium ions, nucleotides, and amino acids to leak out. This rapid and irreversible damage leads to fungal cell death. Its high affinity for ergosterol over cholesterol (the major sterol in human cell membranes) provides its selective toxicity.

The clinical application of nystatin is almost exclusively topical or local due to its significant toxicity and poor absorption when administered orally or intravenously. It is a first-line agent for the treatment of localized Candida infections. Common formulations include oral suspensions or pastilles for oropharyngeal thrush, topical creams and ointments for cutaneous candidiasis, and vaginal tablets for vulvovaginal candidiasis. It is also used as a preservative in cell culture media to prevent fungal contamination and is a component in some combination topical preparations.

As a pharmaceutical, nystatin is not absorbed from intact skin or mucous membranes to a significant degree, which confines its action to the site of application and minimizes systemic exposure. When taken orally for gut decontamination or thrush, it passes through the gastrointestinal tract without being absorbed, acting locally within the lumen. Its formulations are designed for targeted delivery: suspensions must be swished in the mouth for local effect, and creams are applied to affected skin areas. Intravenous formulations are not used due to severe toxicity, including nephrotoxicity and infusion reactions.

The safety profile of topical and orally administered nystatin is excellent, owing to its lack of systemic absorption. Adverse effects are typically mild and localized, such as irritation, burning, or rash at the application site. The oral suspension may cause a bad taste, nausea, or gastrointestinal discomfort. Systemic toxicity is extremely rare with appropriate use. However, its use is contraindicated in individuals with hypersensitivity to nystatin or any component of its formulation. Its long history of safe and effective use has cemented its role as a cornerstone therapy for superficial candidiasis.
