Fusidic Acid Ribosome Recycling Factor Inhibitor: Exerts Bactericidal Activity Against Gram-Positive Pathogens

Fusidic acid is a natural tetratricopene triterpenoid compound isolated from the fermentation broth of the fungus Fusidium coccineum. It belongs to the steroid antibiotics and is the only marketed drug within the fusidic acid antibiotic class. Its chemical formula is C31H48O6, and it appears as a white or off-white crystalline powder. Fusidic acid possesses a unique mechanism of action. It binds to the bacterial elongation factor G (EF-G), inhibiting its dissociation from the ribosome. This action blocks bacterial protein synthesis, exerting bacteriostatic or bactericidal effects. Since this mechanism does not overlap with other commonly used clinical antibiotics, cross-resistance is unlikely to develop. Fusidic acid exhibits strong lipophilicity and excellent tissue penetration, particularly in pathological skin conditions. It readily traverses biological barriers to reach deep infection sites.

(1) Dermatology and Local Infections: This represents fusidic acid's primary therapeutic application. Clinically, it is widely used to treat various skin infections caused by susceptible bacteria (particularly Staphylococcus aureus, including methicillin-resistant Staphylococcus aureus [MRSA]), such as impetigo, boils, furuncles, folliculitis, paronychia, hidradenitis, acne vulgaris, and secondary infections of wounds, eczema, or ulcers. It is commonly formulated as topical preparations such as creams and ointments.

(2) Ophthalmology: Fusidic acid is also used to treat bacterial conjunctivitis and eyelid infections, typically in the form of eye drops (e.g., Fucithalmic).

(3) Systemic Infections: In Europe and other regions, oral formulations (tablets, suspensions) of fusidic acid are used to treat severe staphylococcal infections such as sepsis, pneumonia, osteomyelitis, endocarditis, and bone/joint infections. To delay the development of resistance, loading dose regimens or combination therapy with other antibiotics (e.g., rifampin) are often employed.

(4) Research Applications: As a research tool, fusidic acid is extensively used to investigate bacterial protein synthesis mechanisms and resistance pathways. It also serves as a lead compound for structural modification in developing novel antimicrobial agents. Studies indicate its derivatives exhibit multiple pharmacological activities, including antimalarial, antitubercular, antiviral, and antitumor effects.

It demonstrates a very low minimum inhibitory concentration (MIC) against most Staphylococcus aureus strains.

  (2) Anti-inflammatory and Immunomodulatory Effects: Beyond direct antibacterial action, fusidic acid suppresses inflammatory responses, reducing tissue edema, exudation, and necrosis caused by bacterial infections to alleviate infection symptoms. Studies confirm its direct anti-inflammatory activity.

  (3)Exceptional Tissue Penetration: Fucidin exhibits excellent lipophilicity, enabling it to readily penetrate deep into the skin—even under pathological conditions such as compromised skin barriers—to reach infection sites and effectively eliminate deep-seated bacteria.

  (4) Inhibition of Bacterial Biofilm Formation: Recent studies reveal that fusidic acid and its derivatives disrupt biofilm formation in bacteria like MRSA. For instance, a novel fusidic acid derivative significantly reduces MRSA biofilm formation at subinhibitory concentrations while downregulating the expression of biofilm-associated regulatory factors.
