Tobramycin is an aminoglycoside antibiotic with potent activity against a range of Gram-negative and some Gram-positive bacteria, notably Pseudomonas aeruginosa. It is used in various formulations, including intravenous/intramuscular injection, inhalation, and topical eye preparations.

Mechanism of Action

Tobramycin binds to bacterial 30S (and to some extent 50S) ribosomal subunits, preventing the formation of the functional 70S initiation complex. This blocks mRNA translation, inhibits protein synthesis, and leads to bacterial cell death. It exhibits both immediate bactericidal effects (through membrane disruption) and delayed effects (via impaired protein synthesis).

Clinical Uses

Inhalation: Management of chronic Pseudomonas aeruginosa lung infections in cystic fibrosis patients.
Injection (IV/IM): Treatment of serious infections such as septicemia, meningitis, lower respiratory tract, abdominal, urinary tract, skin, bone, and joint infections caused by susceptible bacteria.
Ophthalmic: Treatment of external eye infections (e.g., conjunctivitis) due to susceptible organisms.

Dosage and Administration

Injection: Usual adult dose is 3 mg/kg/day divided every 8 hours; severe infections may require up to 5 mg/kg/day, with dose reduction as soon as possible. Both IV and IM routes are used, and dosing must be adjusted for renal function. Duration is typically 7–10 days.
Inhalation: Used in cystic fibrosis, dosing and duration depend on patient age and lung function.
Ophthalmic: As directed for topical infections.
