Liraglutide is a long-acting glucagon-like peptide-1 (GLP-1) receptor agonist with 97% sequence homology to native human GLP-1. It is a synthetic 31-amino acid peptide analog modified with a C16 fatty acid side chain (palmitic acid) attached to lysine at position 26 via a glutamic acid spacer. This fatty acid modification enables reversible albumin binding, extending the half-life to approximately 13 hours and supporting once-daily administration.

Liraglutide works by mimicking the GLP-1 hormone released in the gastrointestinal tract in response to eating. It enhances glucose-dependent insulin secretion, suppresses glucagon release, delays gastric emptying, and acts on hypothalamic centers to reduce appetite and increase satiety.
