β-Nicotinamide adenine dinucleotide, commonly known as NAD+, is an important coenzyme existing in living cells. It belongs to dinucleotide, consisting of two nucleotides connected by phosphate groups, including adenine and nicotinamide fragments.

Mechanism of Action

NAD+ acts as a key coenzyme in cellular redox reactions and electron carrier, circulating between oxidized NAD+ and reduced NADH.

Redox reaction participant: It accepts hydride ions in glycolysis, TCA cycle, fatty acid oxidation and oxidative phosphorylation, supporting energy metabolism and ATP generation.
Enzymatic cofactor: Works with dehydrogenase enzymes to sustain cellular respiration and biosynthesis.
Enzymatic substrate: Acts as substrate for sirtuins, PARPs and ADP-ribosyl cyclases, participating in DNA repair, gene expression and calcium signal adjustment.
RNA modification: It may form non-canonical caps at the 5' terminal of RNA to regulate RNA stability.

Biosynthesis and Metabolism

Cells maintain intracellular NAD+ concentration via multiple pathways:

De Novo Synthesis: Synthesized from tryptophan through the kynurenine pathway.
Preiss-Handler Pathway: Produces NAD+ starting from nicotinic acid.
Salvage Pathway: The dominant circulation pathway, recycling NAM, NR and NMN to regenerate NAD+. NAMPT is the rate-limiting enzyme.

Surplus nicotinamide will be methylated and excreted to balance NAD+ concentration.

Physiological Functions

Support glycolysis, TCA cycle and oxidative phosphorylation for energy generation;
Maintain intracellular redox balance;
Participate in DNA repair and genome stability regulation;
Adjust gene expression, immune response and circadian rhythm.

Pharmacokinetics

NAD+ distributes in cytoplasm, mitochondria and nucleus with independent metabolic pools.

Intracellular synthesis and recycling dominate. Direct transmembrane absorption of extracellular NAD+ is limited. NAD+ precursors such as NMN and NR will be metabolized into NAM before participating in NAD+ biosynthesis. NAD+ has short half-life and fast turnover rate.
