    1. Description

Urolithin A (UA; chemical name: 3,8-dihydroxy-6H-dibenzo[b,d]pyran-6-one, also designated 3,8-dihydroxybenzo[c]chromen-6-one) is a naturally occurring gut microbiota-derived metabolite and a member of the urolithin family of polyphenol compounds. It is produced in the human colon through the microbial biotransformation of ellagitannins (ETs) and ellagic acid — polyphenols abundant in pomegranates, walnuts, berries, and certain nuts. Notably, only approximately 40–60% of individuals possess the specific gut bacterial strains required to convert ellagitannins into urolithin A, making direct supplementation a relevant strategy for universal access to this bioactive molecule.

    2. Application

    1. Anti-Aging and Healthy Lifespan Extension

Urolithin A has demonstrated lifespan-extending effects across multiple model organisms, including Caenorhabditis elegans (nematodes) and murine models. By activating mitophagy and improving mitochondrial quality control, UA helps delay the accumulation of dysfunctional mitochondria — a hallmark of cellular aging. Human studies have shown improvements in biomarkers of biological age and physical performance in older adults following UA supplementation.

    2. Mitochondrial Health and Muscle Function

One of the most well-documented applications of UA is the improvement of mitochondrial health in skeletal muscle. Preclinical studies in worms, mice, and humans consistently show that UA enhances mitochondrial respiratory capacity, increases mitochondrial membrane potential, and improves muscle endurance. In healthy older adults, UA supplementation improved muscle strength and exercise capacity, making it relevant for combating age-related sarcopenia and muscle decline.

    3. Cardiovascular Health

Urolithin A has shown cardioprotective effects in both preclinical and clinical settings. In animal models of natural aging and heart failure, UA reduced systolic and diastolic cardiac dysfunction, restored mitochondrial ultrastructural defects, and activated cardiac mitophagy. In a human clinical trial, four months of UA supplementation in healthy older adults significantly reduced plasma ceramide levels — clinically validated biomarkers predictive of cardiovascular disease (CVD) risk. UA also ameliorates obesity-induced metabolic cardiomyopathy via mitophagy activation.

    4. Cognitive Health and Neuroprotection

Emerging research indicates that UA may support brain health through mitophagy activation in neuronal tissue. Studies in mouse models of Alzheimer's disease and accelerated aging (SAMP8) show that early UA intervention prevents age-related hippocampal dysfunction by enhancing mitophagy and mitochondrial function. UA also modulates endoplasmic reticulum (ER)–mitochondria–lysosomal communication via calcium signaling, which is critical for neuronal homeostasis. However, evidence suggests that early intervention is more effective than late-stage treatment for cognitive decline.

    5. Anti-Inflammatory and Antioxidant Applications

Urolithin A exhibits potent anti-inflammatory and antioxidant properties. It scavenges reactive oxygen species (ROS), reduces lipid peroxidation, and modulates inflammatory signaling pathways including NF-κB. These properties make UA relevant for conditions characterized by chronic inflammation and oxidative stress, including metabolic disorders, neurodegenerative diseases, and inflammatory conditions. It has also shown protective effects against D-galactose-induced liver and kidney damage in aging mice through antioxidative, anti-inflammatory, and anti-apoptotic mechanisms.

    3. Main Efficacy

    1. Urolithin A exerts its biological effects through multiple interconnected molecular pathways, with mitophagy activation being its most well-characterized and central mechanism of action.

      1. PINK1/Parkin-Mediated Mitophagy Activation

The primary mechanism by which UA promotes mitochondrial health is through the activation of PINK1 (PTEN-induced kinase 1)/Parkin-mediated mitophagy — the selective autophagic degradation of damaged or dysfunctional mitochondria.

      • PINK1 stabilization: UA stabilizes PINK1 kinase on the outer mitochondrial membrane of damaged mitochondria, where PINK1 normally accumulates when mitochondrial membrane potential (ΔΨm) is dissipated.
      • Parkin recruitment: Stabilized PINK1 recruits the E3 ubiquitin ligase Parkin (PARK2) from the cytosol to the mitochondrial surface.
      • Ubiquitination and phosphorylation: Parkin ubiquitinates outer mitochondrial membrane proteins. PINK1 then phosphorylates the ubiquitin chains at Ser65, creating a positive feedback loop that amplifies the signal.
      • Autophagosome engulfment: Phosphorylated ubiquitin chains recruit autophagy receptors (e.g., p62/SQSTM1, OPTN, NDP52), which bind to LC3 (MAP1LC3B) on the phagophore membrane, leading to engulfment of the damaged mitochondrion.
      • Lysosomal degradation: The autophagosome fuses with a lysosome, where the mitochondrial contents are degraded and recycled.

This process has been demonstrated in C. elegans (where UA enhances expression of mitophagy genes lgg-1, pink-1, and pdr-1), in murine models, and in human cells.

      2. Calcium-Dependent Inter-Organellar Mitophagy Regulation

Recent multi-omic research has revealed that UA reorganizes the endoplasmic reticulum (ER)–mitochondria–lysosomal network to promote mitophagy through calcium signaling:

      • ER calcium release: UA induces calcium (Ca²⁺) release from the endoplasmic reticulum, likely through IP3 receptor activation.
      • Mitochondrial fission: Elevated cytosolic calcium activates DRP-1/DNM1L/DRP1-mediated mitochondrial fission, which segregates damaged mitochondrial segments for targeted degradation.
      • Lysosomal activation: UA enhances lysosomal activity and acidification, improving the degradative capacity of the autolysosome.
      • Calcium dependence: Calcium chelation abolishes UA-induced mitophagy and blocks its beneficial effects on muscle function and lifespan, confirming that calcium signaling is an essential upstream mediator of UA's mitophagy effects.

      3. mTOR Pathway Inhibition

Urolithin A inhibits the mammalian target of rapamycin (mTOR) signaling pathway, a central regulator of cell growth, proliferation, and autophagy. mTOR inhibition:

      • Relieves the suppression of autophagy initiation (ULK1 complex activation)
      • Promotes autophagosome formation
      • Works synergistically with PINK1/Parkin activation to enhance mitophagic flux
      • Contributes to the lifespan-extending effects observed in model organisms

      4. Antioxidant and ROS-Scavenging Activity

The hydroxyl groups at positions 3 and 8 of UA's dibenzopyranone scaffold confer direct free radical-scavenging capacity:

      • Scavenges reactive oxygen species (ROS) including superoxide anion, hydroxyl radicals, and peroxyl radicals
      • Reduces lipid peroxidation and oxidative damage to cellular macromolecules
      • Upregulates endogenous antioxidant defenses (e.g., Nrf2/HO-1 pathway)
      • Reduces mitochondrial oxidative stress by improving mitochondrial function and reducing electron leakage from the respiratory chain

      5. Anti-Inflammatory Signaling Modulation

      UA modulates multiple inflammatory pathways:

      • NF-κB inhibition: Suppresses nuclear factor kappa-light-chain-enhancer of activated B cells (NF-κB) translocation and transcriptional activity, reducing pro-inflammatory cytokine (TNF-α, IL-6, IL-1β) production
      • MAPK modulation: Regulates mitogen-activated protein kinase (MAPK) signaling pathways involved in inflammatory responses
      • Reduction of inflammasome activation: May inhibit NLRP3 inflammasome assembly and caspase-1 activation
      • These effects are partly secondary to improved mitochondrial health, as dysfunctional mitochondria are major sources of pro-inflammatory DAMPs (damage-associated molecular patterns)
