Description

Desmopressin acetate(DDAVP, Stimate) is synthetic 1-desamino-8-D-argininevasopressin. Its efficacy, ease of administration (intranasal),long duration of action, and lack of side effects make itthe drug of choice for the treatment of central diabetesinsipidus. A synthetic analog of the pituitary hormone, ARGININE VASOPRESSIN. Its action is mediated by the VASOPRESSIN receptor V2. It has prolonged antidiuretic activity, but little pressor effects. It also modulates levels of circulating FACTOR VIII and VON WILLEBRAND FACTOR.

Uses

Antidiuretic.Desmopressin has been found to be useful in the treatment of central diabetes insipidus in small animals. It may be useful in treating Von Willebrand’s disease, but its short duration of activity (2 – 4 hours) in this condition, resistance development, and expense limit its usefulness for this disorder. Desmopressin may be useful perioperatively to reduce lymph node involvement and metastatic disease in canine mammary gland cancer.

Drug Function and Efficacy

Similar in structure to the natural hormone arginine vasopressin, the clinical dose has a prolonged duration of action and does not produce pressor side effects. After intravenous injection of 0.3 micrograms per kilogram of body weight, the activity of coagulation factor VIII (VIII:C) in plasma can be increased by 2 to 4 times, the von Willebrand antigen factor (vWF:Ag) can be increased, and tissue-type plasminogen activator (t-PA) can be released at the same time. It can shorten or normalize the prolonged bleeding time caused by uremia, cirrhosis, congenital or drug-induced platelet dysfunction, and unknown causes. The use of this product can avoid the risk of human immunodeficiency virus (HIV) and hepatitis virus infection caused by the use of factor VIII preparations.

Indication	To avoid hyponatremia, water intake should be strictly controlled and the patient's blood sodium level should be monitored in the following situations: - When used in combination with drugs known to cause syndrome of inappropriate antidiuretic hormone secretion (SIADH) (such as tricyclic antidepressants, selective serotonin reuptake inhibitors, chlorpromazine, carbamazepine), these drugs may enhance the antidiuretic effect and increase the risk of fluid retention; - When used in combination with nonsteroidal anti-inflammatory drugs (NSAIDs), these drugs may cause water retention/hyponatremia. The concomitant use of loperamide will cause the plasma concentration of desmopressin acetate to increase by three times, which will increase the risk of water retention/hyponatremia. Although it has not been confirmed, this effect may also occur when used in combination with other drugs that slow bowel movements. The concomitant use of simethicone may reduce the effect of desmopressin acetate.
