Drug Function and Efficacy

Varenicline is a selective partial agonist of the α4β2 subtype of nicotinic acetylcholine receptors, and has a high affinity for this receptor in nerves. Varenicline binds to the α4β2 receptor to produce an agonist effect, while blocking nicotine from binding to the receptor, which is the mechanism by which varenicline exerts its smoking cessation effect. Varenicline can block the agonist effect of nicotine on the α4β2 receptor and the mesolimbic dopamine system, which is the underlying neural mechanism of the reinforcement-reward effect of smoking. Varenicline is highly selective for the α4β2 receptor, and its binding to this receptor subtype is stronger than its binding to other common nicotinic receptors, non-nicotinic receptors and transporters. In addition, varenicline has a moderate affinity for the 5-HT3 receptor (Ki=350nM).
