Milestone Achieved: First TCR-T Therapy Filed for Approval
In a significant development in the field of cancer treatment, Adaptimmune has recently announced the submission of a marketing application for their TCR-T therapy, Afami-cel. This groundbreaking therapy, specifically targeting MAGE-A4, aims to address the unmet clinical needs of patients with synovial sarcoma, a type of soft tissue tumor. If approved, Afami-cel will not only become the first TCR-T therapy on the market but also mark a major milestone in the field of solid tumor immunotherapy.
Afami-cel has obtained priority review status from the U.S. Food and Drug Administration (FDA) after its application was accepted on January 31, 2024. The Prescription Drug User Fee Act (PDUFA) date, which determines the approval decision, is set for August 4 of the same year. This expedited review process reflects the potential impact of Afami-cel in addressing the urgent medical needs of patients with synovial sarcoma.
Synovial sarcoma is a rare type of soft tissue tumor, with an annual incidence of approximately 13,000 cases in the United States alone. Among soft tissue tumors, synovial sarcoma accounts for 5-10% of cases and has a five-year survival rate of only 20%. This highlights the significant clinical demand for effective treatment options in this patient population.
Adaptimmune estimates that the combined sales peak for Afami-cel and lete-cel, another therapy in their pipeline, could reach $400 million. Notably, Afami-cel has demonstrated an objective response rate (ORR) of 39% in second-line treatment, and a two-year survival rate of 70% with a median overall survival (mOS) projected to be 17 months. These promising results surpass historical data, which showed a median OS of less than 12 months.
The successful approval of Afami-cel would mark a significant milestone in cancer treatment, as it represents a shift in targeting tumor antigens from surface/extracellular to intracellular, opening up a new realm of therapeutic possibilities. The approval of Afami-cel would provide a much-needed treatment option for patients with synovial sarcoma, offering hope for improved outcomes and quality of life.
Looking ahead, several Chinese companies, including Xiangxue Pharmaceutical, Tiankeya, Xinghande, Puruijin, Kerui Biotechnology, and Baiji Biotech, have also entered clinical stages with their own TCR-T therapies. This indicates the growing interest and investment in this innovative approach to cancer treatment.
