Function and Efficacy

Berberine hydrochloride and trimethoprim have synergistic antibacterial effects. Berberine hydrochloride has inhibitory effects on a variety of Gram-positive and Gram-negative bacteria in vitro, among which hemolytic streptococci, Staphylococcus aureus, Vibrio cholerae, Neisseria meningitidis, Shigella, Salmonella typhi, Corynebacterium diphtheriae, etc. have stronger inhibitory effects. It also has a certain inhibitory effect on amoeba. Trimethoprim mainly interferes with bacterial folate metabolism, selectively inhibits dihydrofolate reductase, and prevents nucleic acid synthesis. Therefore, the antibacterial effect of the two combined is stronger than that of a single drug, and the drug-resistant strains are reduced.

Appearance

Usage and Dosage

Take 1-3 capsules at a time, 3 times a day.

Adverse Reactions

Nausea, vomiting, drug rash, and diarrhea will disappear after stopping the drug; itching and rash may occur, and occasionally severe exudative erythema multiforme may occur; aseptic meningitis may occasionally occur, with headaches, stiff neck, etc.

Precautions

1. Patients who are allergic to berberine hydrochloride, trimethoprim and sulfonamides are prohibited from using this product. 2. Children with glucose-6-phosphate dehydrogenase deficiency are prohibited from using this product. 3. Newborns and infants under 2 months old are prohibited from using this product.

Special Population Medication

Precautions for children: 1. Berberine hydrochloride can cause hemolytic anemia and lead to jaundice, so it is contraindicated for children with glucose-6-phosphate dehydrogenase deficiency. 2. The trimethoprim in this product may interfere with the folic acid metabolism of breastfeeding infants. Newborns and infants under 2 months old should not use this drug; children are in the growth and development period, and their liver and kidney functions are not yet perfect, so the dosage should be reduced as appropriate. Precautions for pregnancy and lactation: 1. Since trimethoprim can cross the placental barrier, the blood concentration in the fetal circulation and amniotic fluid is close to the maternal blood concentration. Therefore, this product must be used during pregnancy after weighing the pros and cons. 2. Since trimethoprim can be secreted into breast milk, its blood concentration is high, which may interfere with the folic acid metabolism of breastfeeding infants. Although the problem has not been confirmed in humans, the application of this product to breast milk must be decided after weighing the pros and cons. Precautions for the elderly: Elderly patients have reduced resistance and weakened metabolic capacity. When taking this product, they should pay attention to blood tests. If serious adverse reactions occur, the drug should be stopped immediately.

Drug Interactions

1. The interaction between berberine hydrochloride and other drugs is still unclear. 2. Trimethoprim; (1) When used simultaneously with bone marrow suppressants, the chances of leukopenia and thrombocytopenia increase; (2) When used simultaneously with dapsone, the blood concentrations of both drugs increase. The increase in dapsone blood concentration increases and aggravates adverse reactions, especially the occurrence of methemoglobinemia; (3) Anti-tumor drugs, 2,4-diaminopyrimidine drugs should not be used simultaneously. It is also not advisable to use this product between courses of treatment with other folic acid antagonists because of the possibility of bone marrow aplasia or megaloblastic anemia; (4) The simultaneous use of reserpine can increase the elimination of trimethoprim and shorten its plasma half-life; (5) The simultaneous use of cyclosporine can increase renal toxicity; (6) It interferes with the hepatic metabolism of phenytoin and increases the half-life of phenytoin.
