Function and Efficacy

Pharmacological action: This product is used for diagnosis. The sodium ion concentration of its 76% solution is about 136mmol/L, which is close to the concentration in plasma and has little effect on myocardial cell function. Sodium diatrizoate Sodium diatrizoate is an ionic monomeric iodine contrast agent. Iodine can absorb a large amount of X-rays. After being injected into the body, it forms a density contrast with the surrounding tissues under X-rays and develops. When using the direct introduction method for contrast imaging, it can be directly injected into the blood vessels or other cavities to show the luminal morphology. When using the physiological absorption method for contrast imaging, the contrast agent injected into the blood vessels can enter the diseased tissue through the damaged vascular endothelium or the damaged blood-brain barrier to show the lesions. The urinary tract morphology can be displayed when excreted through the kidneys. Diatrizoate meglumine; same as sodium diatrizoate.

Ingredients

This product is a compound preparation, its components are: 1 part of sodium diatrizoate and 6.6 parts of diatrizoate meglumine plus appropriate amount of sodium hydroxide to make a sterile aqueous solution, the total amount of sodium diatrizoate (C11H8I3N2NaO4) and diatrizoate meglumine (C11H9I3N2O4·C7H17NO5) should be 90.0%-110.0% of the labeled amount. Each injection contains 32mg of sodium diatrizoate and 268mg of diatrizoate meglumine.

Appearance

This product is a colorless to light yellow clear liquid. The iodine content of 60% is 292 mg/ml, and the iodine content of 76% is 370 mg/ml.

Indication

It is suitable for urinary tract angiography, cardiac angiography, cerebral angiography, angiography of other organs and peripheral blood vessels, CT enhanced scanning and various other cavity and fistula angiography, and can also be used for coronary angiography.

Usage and Dosage

1. For cardiovascular angiography or aortic angiography, the usual dosage for adults is 40-60 ml (76%), or 1 ml/kg according to body weight, which is injected in about 2 seconds with a pressure syringe. When repeated injections or other angiography are performed simultaneously, the total amount should not exceed 225 ml. The usual dosage for children is 1.0-1.5 ml/kg (76%) according to body weight, and the total amount of repeated injections should not exceed 4 ml/kg. For infants and young children, it should not exceed 3 ml/kg. 2. For coronary angiography, the usual dosage for adults is 4-10 ml (76%) at a time, which can be repeatedly injected and needs to be injected under electrocardiogram monitoring. 3. For cerebral angiography, the usual dosage for adults is 10 ml (60%) at a time, and the injection speed should not exceed 5 ml per second. 4. For limb artery angiography, the usual dosage for adults is 10-40 ml (60%) at a time, which should be injected within 2-3 seconds. 5. Renal artery contrast is injected into the renal artery via a catheter, and the usual dosage for adults is 5-10 ml (60%). 6. Abdominal artery contrast is injected into the abdominal artery via a catheter, and the usual dosage for adults is 30-50 ml (76%), which is injected quickly via a pressure syringe. 7. Lower limb venography is injected through the skin into the dorsal or lateral superficial vein of the foot, and the usual dosage for adults is 20-100 ml (30%-50%). 8. Upper limb venography is injected through the skin into the forearm or hand superficial vein, and the usual dosage for adults is 20-50 ml (30%-50%). 9. CT enhanced scanning is 50-150 ml (60% or 76%), intravenous push or drip. 10. Excretory (intravenous) urography is injected intravenously (conventional method), and the usual dosage for adults is 20-40 ml (60% or 76%). The usual dosage for children is 0.5-1 ml/kg (60% or 76%) according to body weight. Or: 5ml (60%) or 4ml (76%) for children under 6 months; 8ml (60%) or 6ml (76%) for children between 6 and 12 months; 10ml (60%) or 8ml (76%) for children between 1 and 2 years old; 12ml (60%) or 10ml (76%) for children between 2 and 5 years old; 15ml (60%) or 12ml (76%) for children between 5 and 7 years old; 18ml (60%) or 14ml (76%) for children between 7 and 10 years old; 20ml (60%) or 16ml (76%) for children between 10 and 15 years old. 11. The usual dosage for intravenous drip in adults is 2.2ml/kg (60% or 76%) of body weight, add an equal amount of 5% glucose injection solution, and drip rapidly. The speed is slower for the elderly and patients with heart disease. Note: Patients with impaired renal function should not repeat angiography within 48 hours. 12. Retrograde pyelourethrography: 30% is injected slowly through the ureteral catheter. The common dosage for adults is 10-15ml (30%) on one side. 13. Hysterosalpingography: 10ml (76%) is injected through the cervical opening. 14. Intraoperative or postoperative T-tube cholangiography: 10ml (60%). 15. Percutaneous transhepatic cholangiography: 20ml-40ml (60%).

Adverse Reactions

Reactions such as nausea, vomiting, salivation, dizziness, and urticaria may occur.

Precautions

1. Patients who are allergic to iodine are prohibited from using this product. 2. Patients with impaired liver and kidney function, active pulmonary tuberculosis, multiple spinal cord tumors and hyperthyroidism are prohibited from using this product. 3. Patients with homocystinuria should not undergo angiography, otherwise it may cause thrombosis or embolism. 4. This product is strictly prohibited from being injected into the ventricle, intracranial, intraspinal subarachnoid space, cystic cavity and fistula communicating with the subarachnoid space.

Special Population Medication

Precautions for children: This experiment has not been conducted and there is no reliable reference. Precautions for pregnancy and lactation: Please refer to Precautions. Precautions for the elderly: The elderly are more sensitive to the toxic effects of contrast agents and have poor tolerance to the hemodynamic changes caused by high concentrations of contrast agents entering the body. Be careful when using.

Drug Interactions

1. Intravascular injection of this product immediately after taking cholecystography contrast agent will increase the toxic effect on the kidneys, especially in patients with impaired liver function. 2. Although the use of vasopressors during aortic angiography can improve the contrast of the imaging, due to the contraction of visceral blood vessels, a large amount of contrast agent is forced to enter the spinal cord blood vessels, which increases the neurotoxicity of this product and may cause paraplegia. 3. This product should not be mixed with antihistamines for injection. It may precipitate when mixed with promethazine hydrochloride, diphenhydramine hydrochloride, chlorpheniramine maleate (chlorpheniramine), etc.
