Salmon Calcitonin Nasal Spray

Function and Efficacy

Pharmacological action: All calcitonins contain a single chain of 32 amino acids, and the order of its amino acids varies from species to species. Since salmon calcitonin has a greater affinity for the receptor binding site than calcitonin from mammals (including synthetic human calcitonin), it has a stronger and more lasting effect in clinical practice. 1 Calcitonin is the main regulator of mineral and bone metabolism. In maintaining bone mass, it affects the action of parathyroid hormone by affecting the balance of bone and calcium. In diseases with bone resorption and formation, calcitonin significantly reduces bone calcium loss, such as osteoporosis, deforming bone disease, and malignant osteolysis. Inhibit osteoclast activity and stimulate the formation and activity of osteoblasts. Calcitonin inhibits osteolysis, thereby reducing abnormally elevated serum calcium. In addition, it increases the excretion of urinary calcium, phosphorus, and sodium by reducing renal tubular reuptake. However, serum calcium does not drop below the normal range. 2 Calcitonin has analgesic effects, especially in diseases accompanied by bone pain. This effect may act on the central nervous system, because sites that specifically bind to salmon calcitonin have been found in certain areas of the central nervous system. 3. Calcitonin can reduce the secretion of gastric juice and pancreatic exocrine glands. Calcitonin has been shown to be beneficial for the medical treatment of acute pancreatitis. Toxicological studies: 1 Acute toxicity After intravenous injection of salmon calcitonin API (8000ug/kg) and its preparation (750ug/kg) into mice, no toxic reactions were observed in the animals, but one day after the injection, the animals showed significant weight loss, which then gradually recovered. No abnormalities were found in the macroscopic anatomical examination. After subcutaneous injection of salmon calcitonin API (12000ug/kg) and its preparation (750ug/kg) into mice, no obvious toxic reactions were observed in the animals, but one day after the injection, the animals in the preparation group showed significant weight loss, which then gradually recovered; no abnormalities were found in the API group. 2 Chronic toxicity Piglets, rabbits and dogs were injected intramuscularly with salmon calcitonin at 1.78, 5.8 and 19.4ug/kg/day for 6 months. At the beginning of administration, the dogs had slight vomiting, a significant decrease in food and water intake, weight loss, and reduced excretion, which recovered after 6 weeks. 1/6 animals produced antibodies at 3 months. No other changes. Rats were injected subcutaneously with salmon calcitonin at 1.5, 6 and 24ug/kg/day for 6 months. The animals' food, water intake and weight decreased. No other changes.

Appearance

This product is a colorless liquid.

Indication

1. Osteoporosis: (1) Early and late postmenopausal osteoporosis; (2) Senile osteoporosis; (3) Secondary osteoporosis, for example: corticosteroid treatment or lack of activity. In order to prevent progressive bone loss, calcium and vitamin D should be taken in appropriate amounts according to individual needs. 2. Bone pain accompanied by osteolysis and/or osteopenia. 3. Paget’s disease (osteitis deformans), especially patients with the following conditions: (1) Bone pain; (2) Neurological complications; (3) Increased bone turnover, manifested by increased serum alkaline phosphatase and increased urinary hydroxyproline excretion; (4) Progressive spread of bone lesions; (5) Incomplete or recurrent fractures. 4. Hypercalcemia and hypercalcemic crisis caused by the following conditions: (1) Secondary breast cancer, lung or kidney cancer, bone

Usage and Dosage

1 Osteoporosis: 20ug daily or 40ug every other day, once or in divided doses; depends on the condition and the patient's response. 2 Bone pain with osteolysis and/or osteopenia: adjust the dose according to individual needs, 40-80ug daily, 40ug can be given at one time, when a large dose is needed, it should be given in divided doses. 3 Deformed bone disease: 40ug daily, once or in divided doses. 4 Hypercalcemia: Long-term treatment of chronic hypercalcemia-calcemia 40-80ug daily, 40ug can be given at one time, when a large dose is needed, it should be given in divided doses. (The most effective method of administration for hypercalcemia crisis is intravenous drip). 5 Neurodystrophy: Early diagnosis of this disease is very important, and treatment should be started once the diagnosis is confirmed. During 2 to 4 weeks, 40ug is administered once a day, and 40ug can be further administered every other day for up to 6 weeks depending on the clinical situation.

Adverse Reactions

Nausea, vomiting, dizziness, and mild facial flushing with a feeling of warmth may occur. These side effects are dose-related and are more common with intravenous injections than with intramuscular or subcutaneous injections, which are more common than with nasal sprays. Reports of polyuria or rigors are rare. These reactions usually resolve spontaneously, and only in rare cases do they require a temporary dose reduction. In rare cases, administration of salmon calcitonin has resulted in allergic reactions, and individual allergic reactions have been reported to result in tachycardia, hypotension, and collapse. Side effects are less common with nasal sprays.

Precautions

Hypersensitivity to salmon calcitonin (see ADVERSE REACTIONS).

Special Population Medication

Precautions for children: Due to the lack of sufficient data on the long-term use of this drug in children, the treatment period should not exceed several weeks unless the physician deems that long-term treatment is indicated. Precautions for pregnancy and lactation: Salmon calcitonin cannot pass through the animal placenta. However, it is not suitable for pregnant women. It is not recommended to use this drug during lactation because salmon calcitonin can enter breast milk. Precautions for the elderly: No dosage adjustment is required for elderly people.
