Calcium folinate injection

This product is a formyl derivative of tetrahydrofolate, mainly used for the rescue of high-dose methotrexate and other folic acid antagonists. The main function of methotrexate is to bind to dihydrofolate reductase, blocking the conversion of dihydrofolate into tetrahydrofolate, thereby inhibiting DNA synthesis. After entering the body, this product is converted into tetrahydrofolate by tetrahydrofolate reductase, which can effectively counteract the toxic reaction caused by methotrexate, but has no obvious effect on the existing methotrexate neurotoxicity.

The main ingredient of this product is calcium folinate. Its chemical name is N-[4-[(2-amino-5-formyl-1,4,5,6,7,8-hexahydro-4-oxo-6-pteridinyl)methyl]amino]benzoyl-L-glutamic acid calcium salt pentahydrate. Its structural formula is: molecular formula: C20H21CaN7O75H2O, molecular weight: 601.61.

This product is a yellow, clear aqueous solution.

1. Uracil combined use can improve the efficacy of fluorouracil, and is commonly used in the treatment of colorectal cancer and gastric cancer. 2. As an antidote for folic acid antagonists (such as methotrexate, pyrimethamine or trimethoprim). This product is commonly used in the clinic to prevent severe toxic effects caused by high-dose treatment such as methotrexate overdose. 3. For megaloblastic anemia caused by stomatitis, diarrhea, malnutrition, pregnancy or infancy, when oral folic acid is ineffective. It is not suitable for vitamin B12 deficiency anemia.

1 Used for 5-Fu combination enhancement, 20~500mg/m2 each time, intravenous drip, once a day, for 5 consecutive days. It can be diluted with normal saline or glucose injection to make an infusion solution, and the pH of the prepared infusion solution must not be less than 6.5. The infusion solution must be freshly prepared. 2 As a "rescue" therapy for methotrexate, the dosage of this product is best determined based on the blood drug concentration. The generally used dosage is 9mg~15mg/m2 per body surface area, intramuscular or intravenous, once every 6 hours, for a total of 12 times; as an antidote for pyrimethamine or trimethoprim, the dose is 9mg~15mg intramuscularly each time, depending on the poisoning situation.

Rarely, other allergic reactions such as rash, hives, or asthma may occur.

Megaloblastic anemia caused by pernicious anemia or vitamin B12 deficiency.

Precautions for children: Children taking anti-epileptic drugs should use this product with caution (see Drug Interactions). Precautions for pregnancy and lactation: Although animal studies have not shown that this product has reproductive toxicity, animal experiments cannot accurately predict human responses. Since calcium folinate has not been properly and adequately controlled clinical studies in pregnant women, this product should not be used during pregnancy unless absolutely necessary. It is not clear whether this product is secreted in human milk, and lactating women should use this product with caution. Precautions for the elderly: Not yet clear.

The use of this product in large doses together with barbiturates, primidone or phenytoin sodium may affect the anti-epileptic effect.
