Function and Efficacy

This product is the procaine salt of penicillin, and its antibacterial active ingredient is penicillin. Penicillin has good antibacterial activity against hemolytic streptococci and other streptococci, Streptococcus pneumoniae and Staphylococci that do not produce penicillinase. Neisseria gonorrhoeae, Neisseria meningitidis, Corynebacterium diphtheriae, Bacillus anthracis, Actinomyces bovis, Streptobacillus moniliformis, Listeria monocytogenes, Leptospira and Treponema pallidum are sensitive to this product. This product also has antibacterial activity against Haemophilus influenzae and Bordetella pertussis. This product has good antibacterial effect on anaerobic bacteria such as Clostridium, Peptostreptococcus and melanin-producing Bacteroides, but poor antibacterial effect on Bacteroides fragilis. Penicillin exerts its bactericidal effect by inhibiting bacterial cell wall synthesis.

Ingredients

This product is a sterile powder made of procaine penicillin and penicillin sodium (potassium) plus appropriate suspending agents and buffers. Appearance

This product is white powder.

Indication

Due to the low blood concentration of this product, its application is limited to mild and moderate infections caused by pathogens that are highly sensitive to penicillin, such as tonsillitis, scarlet fever, erysipelas, pneumonia caused by group A streptococci, furuncles, carbuncles caused by penicillin-sensitive Staphylococcus aureus, and angina. This product can also be used to treat leptospirosis, relapsing fever and early syphilis.

Usage and Dosage

This product is for intramuscular injection. Before use, add an appropriate amount of sterile water for injection to make a suspension, 400,000 to 800,000 units each time, 1 to 2 times a day.

Adverse Reactions

1. Allergic reaction: urticaria and other types of rashes are common, leukopenia, interstitial nephritis, asthma attacks and serum sickness reactions are less common; anaphylactic shock is occasionally seen. Once it occurs, it must be rescued on the spot, and treatment measures such as keeping the airway open, oxygen inhalation, and adrenaline and glucocorticoids must be given. 2. Herxheimer's reaction and treatment contradictions: When using procaine penicillin to treat syphilis, leptospirosis and other diseases, the symptoms may be aggravated due to the death of pathogens, which is called Herxheimer's reaction; treatment contradictions are also seen in syphilis patients. After treatment, the syphilis lesions disappear too quickly, while tissue repair is relatively slow or the fibrous tissue in the lesion site contracts, hindering organ function. 3. Superinfection: Superinfection with penicillin-resistant Staphylococcus aureus, Gram-negative bacilli or Candida albicans may occur.

Precautions

It is contraindicated in patients with a history of allergy to penicillins or procaine, as well as patients with positive penicillin or procaine skin tests.

Drug Interactions

Probenecid, aspirin, indomethacin, phenylbutazone and sulfonamides reduce the renal tubular secretion of penicillin and prolong the serum half-life of this product. Penicillin can enhance the anticoagulant effect of warfarin.
