Piperacillin Sodium and Tazobactam Sodium for Injection

Function and Efficacy

Piperacillin is a broad-spectrum semisynthetic penicillin antibiotic, and tazobactam is a beta-lactamase inhibitor. This product has antibacterial effects on piperacillin-sensitive bacteria and the following bacteria that produce beta-lactamases and are resistant to piperacillin. 1 Gram-negative bacteria: (1) Most plasmid-mediated beta-lactamase-producing and non-producing bacteria: Escherichia coli, Klebsiella (Klebsiella oxytoca, Klebsiella pneumoniae), Proteus (Proteus mirabilis, Proteus vulgaris), Salmonella, Shigella, Neisseria gonorrhoeae, Neisseria meningitidis, Morganella, Haemophilus (Haemophilus influenzae and Haemophilus parainfluenzae), Pasteurella multocida, Yersinia, Campylobacter, and Gardnerella vaginalis. (2) Chromosome-mediated beta-lactamase-producing and non-producing bacteria: Citrobacter freundii, isocitrifying bacteria, Providencia, Morgenbacterium, Serratia (Serratia marcescens, Serratia hydraulica), Pseudomonas aeruginosa and other Pseudomonas (Pseudomonas cepacia, Pseudomonas fluorescens), Pseudomonas maltophilia, Acinetobacter. 2 Gram-positive bacteria: beta-lactamase-producing and non-producing bacteria: Streptococcus (Streptococcus pneumoniae, Streptococcus pyogenes, Streptococcus bovis, Streptococcus agalactiae, Streptococcus viridans, Group C and Group G Streptococci), Enterococcus (Enterococcus faecalis, Enterococcus faecium), Staphylococcus aureus (excluding MRSA), Staphylococcus saprophyticus, Staphylococcus epidermidis (coagulase-negative staphylococci), Corynebacterium, Listeria monocytogenes, Nocardia. 3 Anaerobic bacteria: the following bacteria that produce and do not produce beta-lactamase: Bacteroides (Bacteroides dipathonis, Bacteroides dimorphus, Bacteroides polytrichoides, Bacteroides melaninogenicus, Bacteroides oralis), Bacteroides fragilis (Bacteroides fragilis, Bacteroides vulgaris, Bacteroides ovipositor, Bacteroides thetaiotaomicron, Bacteroides monothecae, Bacteroides insaccharolyticus), Peptostreptococcus, Clostridium (Clostridium difficile, Clostridium perfringens), Veillonella, and Actinomycetes.

Appearance

Powder for injection is white powder or off-white loose lumps or powder, odorless, bitter and highly hygroscopic.

Indication

Infections caused by sensitive bacteria, such as acute bronchitis, acute exacerbation of chronic bronchitis, pneumonia, lung abscess, bronchiectasis or asthma infection; acute pyelonephritis, acute exacerbation of chronic pyelonephritis, acute cystitis, acute prostatitis, complicated urinary tract infection; appendicitis, peritonitis, intra-abdominal abscess; wound infection, cellulitis, skin abscess, ischemic necrosis and infection of diabetic foot; pelvic infection, endometritis; bone and joint infection, mixed infection with multiple bacteria, bacterial sepsis, toxemia, sepsis.

Usage and Dosage

Adults: Mild infection: 3.375g-4.5g/day, divided into 2-3 intramuscular injections. Moderate infection: 4.5-9g/day, divided into 2-3 intramuscular injections or intravenous drips. Severe infection: 9-13.5g/day, divided into 2-3 intravenous drips.

Adverse Reactions

Rash, itchy skin, diarrhea, nausea, temporary increase in SGPT/SGOT, allergic reactions.

Precautions

Patients with a history of allergy to penicillins, cephalosporins or beta-lactamase inhibitors. Skin test should be done before use, and positive results are contraindicated. Long-term users should have their liver and kidney functions checked regularly. If bleeding occurs, the drug should be discontinued. Use with caution in patients with renal insufficiency, heart failure and hypertension.

Drug Interactions

1 Aminoglycosides: (1) In in vitro tests, this product can inactivate aminoglycosides when used together with them. (2) When this product is used together with tobramycin, the area under the curve, renal clearance and urinary excretion of tobramycin will decrease by 11%, 32% and 38% respectively, because piperacillin/tazobactam may inactivate tobramycin. In patients with severe renal insufficiency, such as hemodialysis patients, the pharmacokinetics of tobramycin will change when it is used in combination with piperacillin. 2 Probenecid: When this product is used in combination with probenecid, the half-life of piperacillin can be extended by 21% and the half-life of tazobactam can be extended by 71%. 3 Vancomycin: When this product is used in combination with vancomycin, the pharmacokinetics are not affected.

Storage

Keep in a dark, sealed and dry cool place
