Glycerol Sodium Chloride Injection
Function and Efficacy
The main pharmacological action is to relax vascular smooth muscle. Nitroglycerin releases nitric oxide (NO), activates guanylate cyclase, increases cyclic guanosine monophosphate (cGMP) in smooth muscle and other tissues, leads to dephosphorylation of myosin light chain, regulates the contraction state of smooth muscle, and causes vasodilation. Nitroglycerin dilates the arteriovenous bed, mainly dilating the veins, and its effect intensity is dose-related. Peripheral vein dilation causes blood to remain in the periphery, reducing the amount of blood returning to the heart and reducing the left ventricular end-diastolic pressure (preload). Dilation of arteries reduces peripheral resistance (afterload). Arteriovenous dilation reduces myocardial oxygen consumption and relieves angina pectoris. It also has a dilating effect on epicardial coronary artery branches. Therapeutic doses can reduce systolic blood pressure, diastolic blood pressure and mean arterial pressure, and the effective coronary perfusion pressure can often be maintained. However, when blood pressure is excessively reduced or heart rate increases, shortening the diastolic filling time, the effective coronary perfusion pressure is reduced. The increased central venous pressure and pulmonary capillary wedge pressure, pulmonary vascular resistance and systemic vascular resistance are reduced. Heart rate is usually slightly increased, presumably as a reflex of the drop in blood pressure. Cardiac index may increase, decrease, or remain unchanged. Patients with increased left ventricular filling pressure and peripheral resistance and low cardiac index may have an increased cardiac index. Conversely, patients with normal left ventricular filling pressure and cardiac index may have a slightly lower cardiac index with intravenous medication.
Ingredients
Each bottle of 500ml contains 50g glycerol and 4.5g sodium chloride. The auxiliary material is water for injection.

Appearance
This product is a colorless clear liquid
Indication
It is used for the treatment and prevention of angina pectoris caused by coronary heart disease, and can also be used to lower blood pressure or treat congestive heart failure.
Usage and Dosage
Injection: Dilute with 5% glucose injection or sodium chloride injection and drip intravenously. The initial dose is 5μg/min. It is best to use an infusion pump to infuse at a constant rate. For lowering blood pressure or treating heart failure, increase by 5μg/min every 3 to 5 minutes. If it is ineffective at 20μg/min, increase by 10μg/min, and then 20μg/min. There are great individual differences in patients' response to this drug. There is no fixed suitable dose for intravenous drip. The dosage should be adjusted according to individual blood pressure, heart rate and other hemodynamic parameters.
Adverse Reactions
(1) Headache: may occur immediately after medication, may be severe and persistent. (2) Dizziness, weakness, palpitations and other manifestations of postural hypotension may occasionally occur, especially in patients who are standing or immobilized. (3) Significant hypotension reactions may occur at therapeutic doses, manifested as nausea, vomiting, weakness, sweating, pallor and collapse. (4) Syncope, flushing, drug rash and exfoliative dermatitis have all been reported.
Precautions
It is contraindicated in patients with early myocardial infarction (severe hypotension and tachycardia), severe anemia, glaucoma, increased intracranial pressure, and known allergy to nitroglycerin. It is also contraindicated in patients using sildenafil citrate (Viagra), which enhances the antihypertensive effect of nitroglycerin.
Drug Interactions
(1) This drug may cause hypotension when used in combination with moderate or excessive alcohol consumption. (2) The postural hypotensive effect of nitrates may be enhanced when used in combination with antihypertensive drugs or vasodilators. (3) Aspirin may reduce the clearance of sublingual nitroglycerin and enhance its hemodynamic effects. (4) The use of long-acting nitrates may reduce the therapeutic effect of sublingual medications. (5) Sildenafil citrate (Viagra) enhances the antihypertensive effect of organic nitrates. (6) The efficacy may be weakened when used in combination with acetylcholine, histamine, and sympathomimetic amines.
