Etoposide
Function and Efficacy
Etoposide is an active ingredient of lignans isolated from podophyllin. VP-16 is a cell cycle-specific anti-tumor drug that acts in the late S phase or G2 phase. Its action site is topoisomerase II, forming a stable cleavable complex between drug, enzyme and DNA, interfering with DNA topoisomerase II, rendering the damaged DNA unable to be repaired. Topoisomerase II inserts into DNA to produce the break reaction required for general cell function; VP-16 appears to cause double-line breaks of DNA and topoisomerase II by stabilizing the DNA break complex. This product activates certain endonucleases in vivo, or acts on DNA through its metabolites, and its non-glycoside homologue 4-demethylepipodophyllotoxin can inhibit microtubule assembly.

Indication
Mainly for small cell lung cancer, the effective rate increased from 40% to 85%, and the complete remission rate was 14% to 34%. For small cell lung cancer, oral administration is more effective than intravenous injection.
Usage and Dosage
Oral administration: Soft capsules, 50 mg each time, 3 times each time, for 5 consecutive days. 21 to 28 days is one cycle, and treatment should be continued for at least 2 cycles.
Adverse Reactions
1. Bone marrow suppression: leukopenia and thrombocytopenia, anemia, which is the dose-limiting toxicity. 2. Gastrointestinal reactions: nausea, vomiting, loss of appetite, stomatitis, diarrhea; occasionally abdominal pain, constipation. 3. Allergic reactions: sometimes allergic reactions such as rash, erythema, itching, etc. may occur. 4. Skin reactions: hair loss is more obvious, sometimes even complete baldness, but it is reversible. 5. Socio-economic toxicity: numbness of hands and feet, headache, etc. 6. Other reactions: fever, abnormal electrocardiogram, hypotension, phlebitis, etc.
Precautions
1. Patients with severe bone marrow suppression and patients with a history of severe allergic reaction to this product are contraindicated. 2. Patients with liver and kidney damage and concurrent infection, and patients with varicella should use this product with caution.
