Sulfadiazine Tablets

Function and Efficacy

Glucose is one of the main sources of heat for the human body. Each 1g of glucose can produce 4 kcal of heat energy, so it is used to supplement heat. Sodium chloride is an electrolyte supplement. Sodium and chloride are important electrolytes in the body, mainly existing in extracellular fluid, and play a very important role in maintaining normal blood and extracellular fluid volume and osmotic pressure.

Appearance

This product is a colorless clear liquid.

Indication

1. Supplement heat and body fluids. Used for insufficiency of food intake or loss of a large amount of body fluids (such as vomiting, diarrhea, etc.) caused by various reasons, total intravenous nutrition, starvation ketosis. 2. Drug diluent. 3. Hypochloremic metabolic alkalosis.

Usage and Dosage

The usage and dosage of glucose and sodium chloride should be considered at the same time. 1. Usage and dosage of glucose 1. Supplementation of heat energy When the patient eats less or cannot eat for some reason, generally 10%-25% glucose injection can be given intravenous drip, and body fluids can be supplemented at the same time. The amount of glucose is calculated according to the required heat energy. 2. Total intravenous nutrition therapy Glucose is the most important energy supply substance for this therapy. In non-protein heat energy, the ratio of glucose to fat heat supply is 2:1. The specific dosage is determined by the clinical calorie requirement. According to the need for fluid replacement, glucose can be prepared into different concentrations of 25% to 50%. Add insulin if necessary, add 1 unit of regular insulin for every 5-10g of glucose. Because this product often uses hypertonic solution

Adverse Reactions

1. Eating less and only supplementing with glucose solution for a long time may cause electrolyte disorders such as hypokalemia and hyponatremia. 2. Overdose of this product in patients with heart failure may cause palpitations, arrhythmia, dyspnea, and even acute left heart failure. 3. Hypophosphatemia is prone to occur when used in combination with insulin without paying attention to phosphate supplementation. 4. Too fast or too much administration may lead to high blood pressure, headache, and dizziness. 5. Too fast or too much administration may lead to weight gain and edema. 6. Phlebitis may occur at the injection site.

Precautions

The following patients are contraindicated for use: 1. Diabetes; 2. Heart failure; 3. Pulmonary edema; 4. Cerebral edema, increased intracranial pressure; 5. Cirrhosis with ascites; 6. Acute renal failure in the oliguric stage; Chronic renal failure with poor response to diuretics; 7. Hypernatremia.

Special Population Medication

Precautions for children: Since sulfonamides can compete with bilirubin for binding sites on plasma proteins, and the acetyltransferase system of newborns is not fully developed, the free blood concentration of sulfonamides increases, which increases the risk of kernicterus. Therefore, this type of drug is contraindicated in newborns and infants under 2 months old. Precautions for pregnancy and lactation: 1. This product can cross the blood-placental barrier to the fetus, and animal experiments have found that it has teratogenic effects. There is a lack of sufficient data for human studies, so pregnant women should avoid using it. 2. This product can be secreted in breast milk, and the concentration in breast milk can reach about 50% to 100% of the maternal blood drug concentration. The drug may have an impact on infants. The use of this product in newborns with glucose-6-phosphate dehydrogenase deficiency may cause hemolytic anemia. In view of the above reasons, this product should not be used by breastfeeding women. Precautions for the elderly: Elderly patients have an increased chance of serious adverse reactions when using this product. Severe rash, bone marrow suppression and thrombocytopenia are common serious adverse reactions in the elderly. Therefore, elderly patients should avoid using it, and when there is a real indication, the decision should be made after weighing the pros and cons.

Drug Interactions

1. The combined use of urine alkalinizing drugs can increase the solubility of this product in alkaline urine and increase excretion. 2. It cannot be used with para-aminobenzoic acid, which can replace this product for bacterial uptake, and the two are antagonistic to each other. It is also not suitable to be used with local anesthetics containing para-aminobenzoyl such as procaine, benzocaine, tetracaine, etc. 3. When used with oral anticoagulants, oral hypoglycemic drugs, methotrexate, phenytoin sodium and thiopental sodium, the above drugs need to adjust the dosage, because this product can replace the protein binding site of these drugs, or inhibit their metabolism, resulting in prolonged drug action time or toxicity. 4. When used with bone marrow suppressive drugs, the potential toxic side effects of such drugs may be enhanced. If there is an indication to use two types of drugs together, the possible toxic reactions should be closely observed. 5. Long-term use with contraceptives (oral estrogen-containing ones) can reduce the reliability of contraception and increase the chance of extramenstrual bleeding. 6. When used with thrombolytic drugs, their potential toxic effects may be increased. 7. When used in combination with hepatotoxic drugs, the incidence of hepatotoxicity may increase. Such patients, especially those who have been taking the drug for a long time and have a history of liver disease, should be closely monitored. 8. When used in combination with photosensitizing drugs, additive photosensitivity may occur. 9. Patients receiving this product have an increased need for vitamin K. 10. It should not be used in combination with methenamine, because methenamine can decompose in acidic urine to produce formaldehyde, which can form insoluble precipitates with this product, increasing the risk of crystalluria. 11. This product can replace the plasma protein binding site of phenylbutazone, and the combination of the two can increase the effect of phenylbutazone. 12. Because this product may interfere with the bactericidal effect of penicillins, it is best to avoid using it at the same time with such drugs. 13. When sulfinpyrazone is used in combination with this product, it can reduce the secretion of this product from the renal tubules, resulting in an increase in blood drug concentration and persistent or toxicity. Therefore, the dose of this product may need to be adjusted during or after the use of sulfinpyrazone.
