Cefazolin Sodium for Injection
Function and Efficacy
Cefazolin is a first-generation cephalosporin with a broad antibacterial spectrum. Except for Enterococcus and methicillin-resistant Staphylococcus, this product has good antibacterial activity against other Gram-positive cocci, and Streptococcus pneumoniae and hemolytic Streptococcus are highly sensitive to this product. Diphtheria Corynebacterium, anthrax Bacillus, Listeria and Clostridium are also very sensitive to this product. This product has good antibacterial activity against some Escherichia coli, Proteus mirabilis and Klebsiella pneumoniae, but has poor antibacterial effect on Staphylococcus aureus. Salmonella typhi, Shigella and Neisseria are sensitive to this product, while other Enterobacteriaceae, Acinetobacter and Pseudomonas aeruginosa are resistant. Enzyme-producing Neisseria gonorrhoeae is resistant to this product; Haemophilus influenzae is only moderately sensitive. Gram-positive anaerobes and some Gram-negative anaerobes are mostly sensitive to this product. Bacteroides fragilis is resistant.

Appearance
This product is white or off-white powder or crystalline powder, odorless.
Indication
It is suitable for the treatment of respiratory tract infections such as otitis media, bronchitis, pneumonia, urinary tract infections, skin and soft tissue infections, bone and joint infections, sepsis, infective endocarditis, hepatobiliary system infections, and eye, ear, nose, and throat infections caused by sensitive bacteria. This product can also be used as a preventive drug before surgery. This product is not suitable for central nervous system infections. The efficacy is poor for chronic urinary tract infections, especially those with urinary tract anatomical abnormalities. This product is not suitable for the treatment of gonorrhea and syphilis.
Usage and Dosage
Common adult dose: slow intravenous push, intravenous drip or intramuscular injection, 0.5-1g at a time, 2-4 times a day. In severe infections, the dose can be increased to 6g a day, administered intravenously in 2-4 times. Common child dose: 50-100mg/kg a day, slow intravenous push, intravenous drip or intramuscular injection in 2-3 times. When the creatinine clearance of patients with renal impairment is greater than 50ml/min, the drug can still be administered at a normal dose. When the creatinine clearance is 20-50ml/min, 0.5g every 8 hours; when the creatinine clearance is 11-34ml/min, 0.25g every 12 hours; when the creatinine clearance is less than 10ml/min, 0.25g every 18-24 hours. The first dose for all patients with varying degrees of renal impairment is 0.5g. When children with renal impairment use cefazolin, first give 12.5 mg/kg, followed by maintenance doses. When the creatinine clearance is above 70 ml/min, normal doses can still be given; when the creatinine clearance is 40-70 ml/min, 12.5-30 mg/kg per body weight every 12 hours; when the creatinine clearance is 20-40 ml/min, 3.1-12.5 mg/kg per body weight every 12 hours; when the creatinine clearance is 5-20 ml/min, 2.5-10 mg/kg per body weight every 24 hours. When this product is used to prevent postoperative infection, it is generally given by intramuscular or intravenous injection of 1 g 0.5-1 hour before surgery. If the operation time exceeds 6 hours, 0.5-1 g is added during the operation, and 0.5-1 g is given every 6-8 hours after surgery until 24 hours after the operation.
Adverse Reactions
1. Thrombophlebitis and pain in the intramuscular injection area after intravenous injection are less frequent and milder than those after cephalothin. 2. The incidence of drug rash is 1.1%, the incidence of eosinophilia is 1.7%, and drug fever may occur occasionally. 3. Some patients may experience temporary elevation of serum aminotransferase and alkaline phosphatase. 4. Patients with renal impairment may experience encephalopathy when using high doses (12g per day) of this product. 5. Superinfection with Candida albicans is occasionally seen.
Precautions
This product is contraindicated in patients who are allergic to cephalosporins or have a history of anaphylactic shock or immediate reaction to penicillin.
Special Population Medication
Precautions for children: This product is not recommended for premature infants and newborns under 1 month old. Precautions for pregnancy and lactation: This product has a low content in breast milk. However, breastfeeding women should still suspend breastfeeding when taking the drug. Precautions for the elderly: The T1/2beta of this product in the elderly is significantly longer than that in young people. The dosage should be appropriately reduced or the dosing interval should be extended according to renal function.
Drug Interactions
1. This product is incompatible with the following drugs and cannot be infused in the same bottle: amikacin sulfate, kanamycin sulfate, chlortetracycline hydrochloride, oxytetracycline hydrochloride, tetracycline hydrochloride, erythromycin gluconate, polymyxin B sulfate, colistin sodium methanesulfonate, pentobarbital, calcium gluconate, calcium gluconate. 2. This product can enhance the antibacterial effect in vitro when used in combination with gentamicin or amikacin. 3. This product may increase nephrotoxicity when used in combination with strong diuretics, and may increase the nephrotoxicity of the latter when used in combination with aminoglycoside antibiotics. 4. Probenecid can increase the blood concentration of this product and prolong its blood half-life.
