Usage and Dosage

Oral administration: Adults take 1 tablet at a time, 2 times a day, not more than 4 times in 24 hours.

Adverse Reactions

The adverse reactions of this product are generally uncommon and usually disappear after continued medication or discontinuation of medication. 1 The more common adverse reactions include diarrhea, dizziness, fatigue, headache and rash. 2 This product has a mild anti-androgen effect. When the dosage is large (more than 1.6g per day), it can cause male breast development, female galactorrhea, loss of libido, impotence, decreased sperm count, etc., which will disappear after discontinuation of medication. 3 This product can pass the blood-cerebrospinal fluid barrier and has certain neurotoxicity. Occasionally, mental disorders are seen, which are more common in the elderly, young children, and seriously ill patients. They can recover within 48 hours after discontinuation of medication. When treating gastrointestinal complications of alcoholics, delirium tremens may occur, which is similar to alcohol withdrawal syndrome; 4 The rare adverse reactions of this product include: allergic reactions, fever, joint pain, myalgia, granulocytopenia, thrombocytopenia, interstitial nephritis, liver toxicity, bradycardia, tachycardia, etc.

Drug Interactions

1. When used in combination with antacids, it can relieve pain in duodenal ulcers, but the absorption of cimetidine may be reduced, so it is generally not recommended. If it must be used in combination with antacids, the two should be taken at least 1 hour apart. 2. Taking metoclopramide (Metoclopramide) with this product at the same time can reduce the blood concentration of this product, and the dose of this product needs to be appropriately increased. 3. Since sucralfate needs to be hydrolyzed by gastric acid to work, this product inhibits gastric acid secretion. The combination of the two may reduce the efficacy of sucralfate. 4. This product inhibits the oxidative metabolic pathway catalyzed by cytochrome P450 and can reduce hepatic blood flow. Therefore, when it is used in combination with other drugs, this product can reduce the metabolism of other drugs, resulting in enhanced pharmacological activity or toxicity. These drugs include: (1) When used in combination with benzodiazepines for a long time, the metabolism in the liver can be inhibited, resulting in increased blood concentration of the latter, aggravated sedation and other central nervous system inhibitory effects, and can develop into respiratory and circulatory failure. However, lorazepam, oxazepam, and temazepam do not seem to be affected. (2) When used in combination with coumarin anticoagulants, the prothrombin time can be further prolonged. Therefore, it is necessary to pay close attention to changes in the condition and adjust the dosage of anticoagulants. (3) When used in combination with phenytoin sodium or other hydantoins, the latter's blood concentration may increase, leading to phenytoin sodium poisoning. If it must be used in combination, the phenytoin sodium blood concentration should be measured after 5 days to adjust the dosage, and pay attention to regular review of peripheral blood. (4) When used in combination with propranolol, metoprolol, and metronidazole, the blood concentration may increase. (5) When used in combination with xanthine drugs such as theophylline, caffeine, and aminophylline, liver metabolism is reduced, which can lead to delayed clearance and increased blood concentration, and toxic reactions may occur. (6) This product can increase the absolute bioavailability of verapamil (Isapamil) from 26.3% plus mn; 16.8% to 49.3% plus mn; 23.6%. Since verapamil can cause rare but serious side effects, it should be used with caution. (7) This product can inhibit the metabolism of quinidine. When patients take digoxin and quinidine at the same time, this product should not be used. Because quinidine can displace digoxin from its binding site, the blood concentrations of quinidine and digoxin will increase. At this time, the blood concentration should be monitored. (8) It should be used with caution when used in combination with other drugs metabolized in the liver, such as lidocaine, tricyclic antidepressants. 5. When used in combination with opioids, it has been reported that adverse reactions such as respiratory depression, mental confusion, and loss of orientation can occur in patients with chronic renal failure. The dosage of opioid preparations should be reduced for such patients. 6 Since this product increases the pH value of gastric juice, when used in combination with tetracycline, the dissolution rate of tetracycline may decrease, absorption may decrease, and the effect may weaken (but the hepatic enzyme inhibitory effect of this product may increase the blood concentration of tetracycline); if used in combination with aspirin, the opposite result will occur, and the effect of aspirin may be enhanced. 7 Combination with ketoconazole may interfere with the absorption of the latter and reduce its antifungal activity, but taking some acidic beverages together can avoid the above changes. 8 Combination of this product with captopril may cause mental symptoms. 9 Since this product has a myoneural blocking effect similar to that of aminoglycoside antibiotics, this effect is not counteracted by neostigmine, but can only be counteracted by calcium chloride. Therefore, when used in combination with aminoglycosides, it may cause respiratory depression or respiratory arrest.
