Function and Efficacy
This product is a new triazole antifungal drug that can specifically and effectively inhibit the sterol synthesis of fungi. Oral and intravenous administration of this product is effective for various animal fungal infections: such as Candida infection, Cryptococcus neoformans infection, Microsporum infection, Trichophyton, Epidermophyton, Pityrosporum and other infections. Pharmacokinetics This product is well absorbed orally, and the bioavailability can reach more than 90% of intravenous administration. After 1-2 hours of oral administration, the blood drug concentration generally reaches the peak, and the plasma elimination half-life is about 30 hours. The blood drug concentration is proportional to the dose. The plasma protein binding rate is low, generally 11-12%. This product can penetrate into body fluids well, such as the drug concentration in cerebrospinal fluid is about 80% of the blood drug concentration. 80% of the dose of this product is excreted in the urine as the prototype drug.

Indication
1. Candidiasis; 2. Cryptococcosis; 3. Coccidioidomycosis; 4. For the prevention of Candida infections in patients receiving chemotherapy, radiotherapy and immunosuppressive therapy; 5. This product can also replace itraconazole for the treatment of blastomycosis and histoplasmosis; 6. It can also be used for superficial fungal diseases such as tinea manuum, tinea corporis, tinea versicolor, and onychomycosis.
Usage and Dosage
Oropharyngeal candidiasis in adults: 200 mg for the first dose, followed by 100 mg/day for 7-14 consecutive days. Other mucosal candidiasis infections, such as esophagitis, pulmonary bronchial infections, candiduria, and skin and mucosal candidiasis: 200 mg for the first dose, followed by 100 mg/day for 14-30 consecutive days. In severe cases, the dose can be increased to 200 mg/day. Candidal sepsis, disseminated candidiasis, and other non-superficial candidiasis infections: 400 mg for the first dose, followed by 200 mg/day. Depending on the clinical response, the daily dose can be increased to 400 mg. The course of treatment depends on the clinical situation. In severe cases, it can be used in combination with other antifungal drugs. Vulvar and vaginal candidiasis: 150 mg as a single dose. Cryptococcal meningitis and cryptococcal infections in other parts: usually the first dose is 400 mg, followed by 200-400 mg/day, generally at least 8-12 weeks, and the course of treatment depends on the clinical and mycological response after taking the medicine. Superficial fungal diseases such as tinea pedis, tinea corporis and cruris: 150 mg/week for 3-4 consecutive weeks. Tinea unguium 150 mg/week each time for 12-16 consecutive weeks, and the course of treatment can be appropriately extended depending on the condition. The preventive treatment dose is 50-100 mg, once a day. For patients with renal insufficiency, when multiple doses are given, the dosage or the interval between doses should be adjusted. For children who are considered to have to use this product for treatment: superficial candida infection: 1-2 mg/kg/day; systemic candida/cryptococcal infection: 3-6 mg/kg/day.
Adverse Reactions
Patients generally tolerate this product well. Common adverse reactions include nausea, abdominal pain, diarrhea, flatulence, rash, and occasional elevated alanine aminotransferase. Contraindications Patients with a history of allergy to this product or other triazole drugs should not use it.

Special Population Medication
Precautions for children: There is a lack of sufficient research data on the effects of this product on children. Although a small number of pediatric patients aged 2 weeks to 14 years old have not experienced adverse reactions when treated with a daily dose of 3-6 mg/kg (by body weight), it is still not suitable for children. Precautions for pregnancy and lactation: 1. In animal experiments, when this product is given to animals at high doses, miscarriage, increased stillbirths, rib deformities in young animals, cleft palate and other changes may occur. Although such conditions have not been found in humans, pregnant women should still not use it. 2. There is no data on the concentration of this product in breast milk, so lactating women should use it with caution or stop breastfeeding when taking this product. Precautions for the elderly: Elderly patients with no renal function impairment do not need to adjust the dose. Elderly patients with renal impairment should adjust the dose according to creatinine clearance (see [Usage and Dosage] for details).
Drug Interactions
This product can prolong the prothrombin time when used with coumarin drugs, so the dosage of anticoagulants should be adjusted carefully. This product can be taken with oral sulfonylureas, but hypoglycemia may occur. Patients who take multiple doses of hydrochlorothiazide and this product at the same time will have their fluconazole blood concentration increased by 40%, so attention should be paid. Taking this product and phenytoin at the same time can increase the concentration of phenytoin to a clinically significant level. If both are to be taken at the same time, the concentration of phenytoin must be monitored and its dosage adjusted to the maintenance treatment level. Taking this product and rifampicin at the same time will reduce rifampicin by 25% and shorten the half-life of fluconazole by 20%, so the dosage of this product should be considered to be increased.
