Nimodipine is a Ca2 channel blocker. Under normal circumstances, the contraction of smooth muscle depends on Ca2 entering the cell, causing the depolarization of the transmembrane current. Nimodipine achieves the purpose of relieving vasospasm by effectively preventing Ca2 from entering the cell and inhibiting smooth muscle contraction. Animal experiments have shown that the effect of nimodipine on cerebral arteries is much stronger than that on arteries in other parts of the body, and because it has a high lipophilic characteristic, it can easily pass through the blood-brain barrier. When used to treat subarachnoid hemorrhage, the concentration in cerebrospinal fluid can reach 12.5ng/ml. It can be inferred that it can be used clinically to prevent vasospasm after subarachnoid hemorrhage, but the mechanism of action of this drug in humans is still unclear. The main ingredient of this product is nimodipine.

Light yellow tablets, odorless and tasteless. Not stable to light, but relatively stable to moisture and heat.

It is suitable for cerebral vasospasm after subarachnoid hemorrhage of various causes and blood circulation improvement in the recovery period of acute cerebrovascular disease. Calcium antagonists are used for ischemic cerebrovascular disease, migraine, cerebral vasospasm caused by mild subarachnoid hemorrhage, sudden deafness, and mild to moderate hypertension.

1 Ischemic cerebrovascular disease: Oral. 30-120 mg per day, divided into 3 times, for one month. 2 Migraine: Oral. 40 mg once, 3 times a day, 12 weeks as a course of treatment, the effective rate is 88%, about half of the cases can be basically cured or significantly effective, it can reduce the pain degree, frequency and duration of attacks, and prevent the appearance of premonitory symptoms for vascular, tension, cluster and mixed headaches. 3 Cerebral vasospasm caused by subarachnoid hemorrhage: Oral. 40-60 mg once, 3-4 times a day, 3-4 weeks as a course of treatment. For patients who need surgery, stop taking the medicine on the day of surgery, and continue to take it later. 4 Sudden deafness: Oral. 40-60 mg per day, divided into 3 times, 5 days as a course of treatment, generally 3-4 courses of treatment. 5 Mild and moderate hypertension: Patients with hypertension and cerebrovascular disease can be given priority. Oral. Start with 40 mg once, 3 times a day, the maximum daily dose is 0.24 g.

A large number of clinical practices have shown that about 11.2% of patients with subarachnoid hemorrhage experience adverse reactions when treated with nimodipine. The most common adverse reactions are: (1) decreased blood pressure, the degree of which is related to the drug dose. (2) hepatitis. (3) skin irritation. (4) gastrointestinal bleeding. (5) thrombocytopenia. (6) occasional transient dizziness, headache, facial flushing, vomiting, gastrointestinal discomfort, etc.

When nimodipine 90 mg/day is used in combination with cimetidine 1000 mg/day for more than one week, the blood concentration of nimodipine can increase by 50%, which may be related to the inhibition of nimodipine metabolism by cimetidine in the liver cytochrome P450.
