Peritoneal dialysis fluid (lactate)
Function and Efficacy
Peritoneal dialysis uses the peritoneum as a semipermeable membrane. Water and solutes are exchanged between the peritoneal capillaries and the dialysate. Electrolytes and small molecules move from the high concentration side to the low concentration side (diffusion), while water molecules move from the low osmotic concentration side to the high osmotic concentration side (osmosis). Increasing the dialysate concentration can achieve the purpose of removing water from the body. Through the solute concentration gradient difference, uremic substances in the blood can be removed from the dialysate, and electrolyte and acid-base balance can be maintained, replacing part of the kidney's function. The basic principles of peritoneal dialysis fluid formulation: (1) The water used for the dialysate must be strictly sterile and endotoxin-free. (2) The electrolyte concentration of the dialysate is close to that of normal plasma and can be adjusted according to clinical conditions. In this dialysate: ① The sodium ion concentration is 132mmol/L, slightly lower than positive.

Indication
1. Acute renal failure. 2. Chronic renal failure. 3. Acute drug or poisoning. 4. Refractory heart failure. 5. Refractory edema. 6. Electrolyte imbalance and acid-base imbalance.
Usage and Dosage
For the treatment of acute and chronic renal failure with water retention, intermittent peritoneal dialysis is used, 2L each time, retained for 1 to 2 hours, and exchanged 4 to 6 times a day. For those without water retention, continuous ambulatory peritoneal dialysis (CAPD) is used, generally 4 times a day, 2L each time, with an interval of 4 to 5 hours during the day and 9 to 12 hours at night, to increase the clearance of middle molecular uremic toxins. The average daily dialysate volume is 8L.
Adverse Reactions
Common adverse reactions of peritoneal dialysis fluid include: 1. Dehydration 2. Hypokalemia 3. Hyperglycemia 4. Hyponatremia, hypochloremia, metabolic alkalosis 5. Chemical peritonitis.
Precautions
1Extensive intestinal adhesion and intestinal obstruction. 2Severe respiratory insufficiency. 3Extensive abdominal skin infection. 4Abdomen.
